医防融合管理模式在中老年慢性病患者疾病控制中的应用

Application of Medical and Prevention Integration Management Mode in the Disease Control of Middle-Aged and Elderly Patients With Chronic Diseases

目的通过探讨医防融合管理模式对中老年慢性病患者疾病控制水平的影响,评估管理模式对其效果,进而有效应用于实践中。方法抽取2021年3月—2022年8月宿迁地区的2146例慢性病患者为研究对象。按不同管理模式分为观察组与对照组,各1073例。对照组患者普通门诊随诊管理模式,观察组患者医防融合管理模式。比较两组管理前后的血压、血糖、身体质量指数、低密度脂蛋白胆固醇、三酰甘油水平以及对疾病认知程度。结果管理前,两组患者疾病认知程度比较,差异无统计学意义(P>0.05);管理后,两组患者疾病认知程度各项均有所提升,且观察组疾病认知程度各项均高于对照组,差异有统计学意义(P<0.05)。管理前,两组患者血压、血糖、身体质量指数、低密度脂蛋白胆固醇以及三酰甘油水平比较,差异无统计学意义(P>0.05)。管理后,两组患者血压、血糖、身体质量指数、低密度脂蛋白胆固醇以及三酰甘油水平皆有所降低,且观察组血压、血糖、身体质量指数、低密度脂蛋白胆固醇以及三酰甘油水平均低于对照组,差异有统计学意义(P<0.05)。结论医防融合管理模式应用于中老年慢性患者疾病管理中效果非常显著,不仅可以提升患者对疾病认知程度,还可以明显降低患者血压、血糖、身体质量指数、低密度脂蛋白胆固醇以及三酰甘油水平,有利于延缓患者病情发展,值得推广及实践。本研究结果为中老年慢性病患者后期管理标准制定提供了借鉴内容。

Objective By discussing the influence of the medical and prevention integration management mode in the disease control level of middle-aged and elderly patients with chronic diseases,evaluating the effect of the management mode,and then effectively applied in practice.Methods A total of 2146 patients with chronic diseases in Suqian from March 2021 to August 2022 were selected as the study subjects.The observation group and the control group were divided into 1073 cases by different management modes.The patients in the control group were given the general outpatient follow-up management mode,and the patients in the observation group were given the medical and prevention integration management mode.The blood pressure,blood glucose,body mass index,low density lipoprotein cholesterol,triglyceride levels and the degree of disease cognition of the two groups were compared before and after management.Results Before management,there was no statistically significant difference in disease cognition between the two groups(P>0.05).After management,the disease cognitive level of patients in the two groups was improved,and the disease cognitive level in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Before management,there was no significant difference in blood pressure,blood glucose,body mass index,low density lipoprotein cholesterol and triglyceride levels between the two groups(P>0.05).After management,the levels of blood pressure,blood glucose,body mass index,low density lipoprotein cholesterol and triglyceride in the two groups decreased,and the levels of blood pressure,blood glucose,body mass index,low density lipoprotein cholesterol and triglyceride in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion The application of the medical and prevention integration management mode in the management of chronic diseases in middle-aged and elderly patients is very effective.It can not only improve the patients’awareness of the disease,but also significantly reduce the patients’blood pressure,blood sugar,body mass index,low density lipoprotein cholesterol and triglyceride level,which is conducive to delaying the development of patients’disease,and is worthy of promotion and practice.The research results of this paper provide references for the formulation of late management standards for middle-aged and elderly patients with chronic diseases.
