2型糖尿病合并良性前列腺增生患者术后性功能障碍的影响因素分析

Influencing factors of postoperative sexual dysfunction in patients with type 2 diabetes mellitus complicated with benign prostatic hyperplasia

目的探讨2型糖尿病(T2DM)合并良性前列腺增生(BPH)患者术后性功能障碍的影响因素。方法选取2018年1月至2020年6月广西梧州市红十字会医院诊治的140例T2DM合并BPH患者作为研究对象。以患者术后1个月的性功能障碍情况划分为障碍组、正常组,比较两组的基线资料,采用多因素Logistic回归分析T2DM合并BPH患者术后性功能障碍的影响因素。结果共90例术后发生性功能障碍(障碍组),50例术后未发生性功能障碍(正常组)。障碍组的年龄大于正常组,障碍组的糖尿病病程长于正常组,障碍组的下尿路症状(LUTS)≥2项占比多于正常组,障碍组的雌二醇(E 2)水平高于正常组,障碍组的睾酮(T)水平低于正常组,障碍组的糖化血红蛋白(HbA 1C)水平高于正常组,差异均具有统计学意义(P<0.05)。多因素Logistic回归分析结果表明:高龄、糖尿病病程长、LUTS症状多、E 2高表达、T低表达、HbA 1C高表达均是T2MD合并BPH患者术后发生性功能障碍的影响因素(P<0.05)。结论T2DM合并BPH患者术后发生性功能障碍与其年龄、糖尿病病程、LUTS症状及E 2、T、HbA 1C指标有关,临床应针对相关指标采取措施加强干预。

Objective To investigate the influencing factors of postoperative sexual dysfunction in patients with type 2 diabetes mellitus(T2DM)complicated with benign prostatic hyperplasia(BPH).Methods A total of 140 patients with T2DM complicated with BPH who received surgical treatment from January 2018 to June 2020 in Guangxi Wuzhou Red Cross Hospital were selected as the research objects.The patients were divided into dysfunction group and normal group according to their sexual dysfunction 1 month after operation.The baseline data of the two groups were compared.Multivariate Logistic regression was used to analyze the influencing factors of postoperative sexual dysfunction in patients with T2DM complicated with BPH.Results A total of 90 patients had postoperative sexual dysfunction(dysfunction group),and 50 patients had no postoperative sexual dysfunction(normal group).The age of the dysfunction group was older than the normal group,the course of diabetes in the dysfunction group was longer than the normal group,the proportion of lower urinary tract symptoms(LUTS)≥2 in the dysfunction group was higher than the normal group,the level of estradiol(E 2)of the dysfunction group was higher than the normal group,the level of testosterone(T)of the dysfunction group was lower than the normal group,the level of glycosylated hemoglobin(HbA 1C)in the dysfunction group was higher than the normal group,and differences above were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that advanced age,diabetes with longer course,combined with more LUTS symptoms,higher expression of E 2,lower expression of T and higher expression of HbA 1C were the influencing factors of postoperative sexual dysfunction in patients with T2MD complicated with BPH(P<0.05).Conclusions The incidence of postoperative sexual dysfunction in patients with T2DM complicated with BPH is related to age,course of diabetes,LUTS symptoms and E 2,T,HbA 1C indicators.Clinical measures should be taken to strengthen intervention for relevant indicators.
