左炔诺孕酮宫内缓释系统在子宫内膜息肉术后辅助治疗中的效果观察

The role of LNG-IUS in adjuvant treatment of endometrial polyps after operation

目的 探讨左炔诺孕酮宫内缓释系统(levonorgestrel releasing intrauterine system,LNG-IUS)在子宫内膜息肉术后辅助治疗中的临床效果。方法 选取2016年1月至2020年12月于首都医科大学附属北京同仁医院经手术确诊的子宫内膜息肉患者150例,根据子宫内膜息肉电切术后采取治疗方式的不同,分为LNG-IUS组(78例)及对照组(72例)。LNG-IUS组在第1个月经周期的7d内放置LNG-IUS,对照组在宫腔镜电切术后不采取任何预防复发的措施。术后12个月随访患者的月经改善情况及子宫内膜息肉有无复发。结果 术后12个月两组患者的月经情况均较术前有不同程度的改善,但LNG-IUS组在月经周期改善、经期改善及经量改善等方面均较对照组更为显著,差异有显著性(P<0.05)。术后12个月随访时LNG-IUS组无复发,对照组9例出现复发,复发率为12.5%,明显高于LNG-IUS组,差异有显著性(χ^(2)=8.274,P=0.004)。结论 LNG-IUS在子宫内膜息肉术后辅助治疗中具有显著的临床效果,能够改善月经情况并预防子宫内膜息肉的复发。

Objective To explore the role of LNG-IUS in the adjuvant treatment of endometrial polyps after operation.Method 150 patients with endometrial polyps diagnosed by surgery in our hospital from January 2016 to December 2020 were selected,including 78 in the LNG-IUS group and 72 in the control group.LNG-IUS group was placed within 7 days after hysteroscopy.The patients were followed up 12 months after operation for menstrual improvement and recurrence of endometrial polyps.Result The improvement of menstruation in LNG-IUS group was significantly higher than that in the control group(P<0.05).The recurrence rate of endometrial polyps in the LNG-IUS group was significantly lower than that in the control group(χ^(2)=8.274,P=0.004).Conclusion LNG-IUS has a significant role in the adjuvant treatment of endometrial polyps,which can improve menstruation and prevent the recurrence of endometrial polyps.
