摘要
目的探讨基于计划(plan)→执行(do)→检查(check)→处理(action)(PDCA)模式的案例教学法(case-based learning,CBL)在神经外科住院医师规范化培训的临床教学效果。方法选取2020年1月—2022年2月216名在武汉大学人民医院神经外科接受住院医师规范化培训的住院医师,随机平均分为对照组和观察组,对照组采用传统教学方法,观察组采用PDCA式CBL教学模式,采用问卷调查及综合考评的方法评估两种不同方法的教学效果。结果理论考试成绩显示观察组学生的成绩(91.83±5.65)分高于对照组(80.02±6.40)分,临床操作考试成绩显示观察组学生的临床实践能力测试成绩(86.00±6.77)分高于对照组(75.89±5.56)分,差异均有统计学意义(P<0.001);问卷调查结果显示观察组学生的教学认可度和满意度均高于对照组,差异有统计学意义(P<0.01)。结论PDCA式CBL教学模式有利于培养学生的学习兴趣,提高住院医师的临床实践能力,提高临床课程的教学质量,在神经外科住院医师规范化培训的教学过程中具备一定的优势。
Objective To explore the clinical teaching effect of case-based learning(CBL)teaching method combined with Plan-Do-Check-Action(PDCA)cycle mode in the standardized training for neurosurgery residents.Methods A total of 216 residents who received standardized training in department of neurosurgery of Renmin Hospital of Wuhan University from January 2020 to February 2022 were randomly divided into the control group and the observation group.The traditional teaching method was adopted in the control group while the PDCA CBL teaching mode was adopted in the observation group.The questionnaire survey and comprehensive evaluation method were used to evaluate the teaching effect of the two different teaching methods.Results The results of theoretical examination showed that the score of students in the observation group(91.83±5.65)points was higher than that in the control group(80.02±6.40)points,and the results of clinical operation test showed that the score of students in the observation group(86.00±6.77)points was higher than that in the control group(75.89±5.56)points,and the differences were statistically significant(P<0.001).The results of questionnaire survey showed that the teaching recognition and satisfaction of students in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.01).Conclusion CBL combined with PDCA cycle mode is beneficial to cultivate students'interest in learning,improve students'clinical practice ability and the teaching quality of clinical courses.It has certain advantages in standardized training for neurosurgery residents.
