柴术理胃饮治疗功能性消化不良肝郁脾虚证的疗效研究

Study on the Curative Effect of Chaizhu Liwei Decoction in the Treatment of Functional Dyspepsia with Liver Depression and Spleen Deficiency Syndrome

目的探讨柴术理胃饮对肝郁脾虚型功能性消化不良(functional dyspepsia,FD)患者症状、焦虑、抑郁及睡眠质量的改善。方法将笔者医院收治的80例肝郁脾虚型FD患者随机分为观察组和对照组,各40例。观察组给与柴术理胃饮治疗,对照组给与多潘立酮片治疗,两组疗程均为4周。治疗前后对FD症状评分并记录症状消失时间;进行临床疗效评价;比较治疗前后汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)、汉密尔顿抑郁量表(Hamilton depression scale,HAMD)、匹兹堡睡眠质量量表(Pittsburgh sleep quality index,PSQI)评分的变化,并进行安全性观察。结果治疗后,观察组餐后腹胀、上腹痛、上腹部烧灼感症状评分,焦虑、抑郁及睡眠质量总分均优于对照组(P<0.05);治疗后,观察组除早饱感外,其余症状消失时间明显短于对照组(P<0.05);临床疗效观察组高于对照组(87.50%vs 65.00%,P<0.05);安全无不良反应。结论柴术理胃饮治疗肝郁脾虚型FD可有效调节患者的焦虑、抑郁情绪,改善睡眠质量及相关症状,并缩短症状消失时间,疗效安全。

Objective To investigate the effect of Chaizhu Liwei Decoction on symptoms,anxiety and depression and sleep quality in patients with functional dyspepsia(FD)of liver depression and spleen deficiency type.Methods 80 FD patients with liver depression and spleen deficiency in our hospital were randomly divided into the observation group and the control group,with 40 patients in each group.The observation group was treated with Chaizhu Liwei Yin,and the control group was treated with domperidone tablets.Both groups were treated for 4 weeks.FD symptom score and symptom disappearance time were recorded before and after treatment;clinical efficacy was evaluated;the scores of Hamilton Anxiety Scale(HAMA),Depression Scale(HAMD)and Pittsburgh Sleep Quality Index(PSQI)were compared before and after treatment;and safety of the treatment was observed.Results After treatment,the scores of abdominal distension,upper abdominal pain,upper abdominal burning sensation,anxiety,depression and sleep quality in the observation group were better than those in the control group(P<0.05).After treatment,the disappearance time of symptoms in the observation group was significantly shorter than that in the control group except for early satiety(P<0.05).The clinical efficacy of the observation group was higher than that of the control group(87.50%vs 65.00%,P<0.05).No adverse reactions were observed.Conclusion Chaihu Liwei Decoction can effectively regulate anxiety and depression,improve sleep quality and related symptoms,and shorten the disappearance time of symptoms in patients with FD of liver depression and spleen deficiency.
