生育舞蹈联合立式分娩在初产妇阴道分娩中的临床研究

The clinical study of birth dance combined with vertical delivery in vaginal delivery of primiparas

目的 探讨生育舞蹈联合立式分娩在初产妇阴道分娩中的临床疗效。方法 纳入2020年8月-2021年5月新余市妇幼保健院产科拟行阴道分娩的200例初产妇作为研究对象,采用非随机对照研究方法,在知情同意的前提之下,按照产妇自身意愿分为2组,对照组采取传统体式分娩,观察组采取生育舞蹈联合立式体位分娩。观察2组产程时间,产后2 h平均出血量,疼痛评分,新生儿健康状况,剖宫产率、会阴侧切率。结果 观察组第一产程、第二产程时间短于对照组,差异显著(P<0.05);2组第三产程时间对比,差异不显著(P>0.05)。观察组产后2 h平均出血量为(279.45±11.64)mL,低于对照组的(313.57±12.85)mL,差异显著(P<0.05)。观察组临产时、第二产程开始时、胎儿娩出前VAS评分低于对照组,差异显著(P<0.05);观察组新生儿Apgar评分为(9.75±0.42)分,和对照组的(9.59±0.44)分对比,差异不显著(P>0.05);观察组剖宫产率、会阴侧切率低于对照组,差异显著(P<0.05)。结论 生育舞蹈联合立式分娩在初产妇阴道分娩中的效果较好,可以缩短产程时间,降低产后出血量,减轻疼痛,且不影响新生儿的健康状况,还可以明显降低剖宫产率、会阴侧切率。

Objective To explore the clinical effect of birth dance combined with vertical delivery in vaginal delivery of primiparas.Methods 200 primiparas who planned to have vaginal delivery from August 2020 to may 2021 were included in this study.Non randomized controlled study method was used.Under the premise of informed consent of patients,the subjects were divided into two groups according to the wishes of pregnant women.Control group:traditional delivery.Observation group:childbirth dance combined with vertical position delivery.Observe the labor process time of primiparas;The average amount of bleeding 2 hours after delivery of primiparas;Pain score of primipara;Determination of neonatal health status;Cesarean section rate and perineal lateral resection rate.Results The first stage of labor and the second stage of labor in the observation group were shorter than those in the control group,and there was significant difference between the data(P<0.05);There was no significant difference in the third stage of labor between the two groups(P>0.05);The average amount of postpartum hemorrhage in the observation group was(279.45±11.64)mL,which was lower than that in the control group(313.57±12.85)mL.the difference between the data was significant(P<0.05).VAS scores of the observation group at the time of labor,at the beginning of the second stage of labor,and before delivery of the fetus were lower than those of the control group at the same time,and there were significant differences between the data(P<0.05);The Apgar score of newborns in the observation group was(9.75±0.42),which was not significantly different from that of the control group(9.59±0.44)(P>0.05).The cesarean section rate and the total incidence of perineum lateral resection rate in the observation group were lower than those in the control group,and the difference between the data was significant(P<0.05).Conclusion The effect of birth dance combined with vertical delivery in vaginal delivery of primiparas is better,which can shorten the labor process time of primiparas,reduce the average bleeding volume in 2 hours after delivery,reduce the pain of primiparas,do not affect the health status of newborns,and significantly reduce the cesarean section rate and perineal lateral resection rate.
