肌内注射双氯芬酸钠利多卡因的不良反应分析

Analysis of Adverse Reactions of Diclofenac Sodium and Lidocaine Intramuscular Injection

目的:分析肌内注射双氯芬酸钠利多卡因引起不良反应的原因,探讨防范的护理对策。方法:回顾性分析2021年1月—2022年6月于广西体育运动创伤专科医院门诊行肌内注射双氯芬酸钠利多卡因的300例患者临床资料,统计全身不良反应和注射部位不良反应发生率。结果:300例患者出现上腹胀痛12例,恶心呕吐5例,头晕眼花8例,头痛5例,出疹瘙痒8例,全身不良反应发生率为12.67%;出现注射部位红肿6例,硬结136例,注射部位不良反应发生率为47.33%。臀部肌肉硬结患者治疗1个疗程出现硬结22例,硬结发生率为16.18%,2个疗程出现硬结114例,硬结发生率为83.82%。治疗2个疗程患者注射部位硬结发生率高于1个疗程患者,差异有统计学意义(P<0.0001)。结论:肌内注射双氯芬酸钠利多卡因可引起患者许多全身不良反应和注射部位不良反应,需采取针对性防范的护理措施对策,以降低不良反应发生率,保障患者的用药安全。

Objective:To analyze the causes of adverse reactions caused by intramuscular injection of diclofenac sodium and lidocaine,and explore the preventive nursing countermeasures.Methods:The clinical data of 300 patients with intramuscular injection of diclofenac sodium and lidocaine in the Outpatient Department of Guangxi Sports Trauma Special Hospital from January 2021 to June 2022 were retrospectively analyzed.The incidence of systemic adverse reactions and adverse reactions at the injection site were statistically analyzed.Results:Among the 300 patients,12 patients had abdominal distension and pain,5 patients had nausea and vomiting,8 patients had dizziness,5 patients had headache,8 patients had rash and itching,and the incidence of systemic adverse reactions was 12.67%;6 patients had swelling at the injection site and 136 patients had induration at the injection site,and the incidence of adverse reactions at the injection site was 47.33%.Among the patients with bottocks muscle induration,22 patients had induration in one course of treatment,and the incidence of induration was 16.18%;114 patients had induration in two courses of treatment,and the incidence of induration was 83.82%.The incidence of induration at the injection site in two courses of treatment was higher than that in one course of treatment,and the difference was statistically significant(P<0.0001).Conclusion:Intramuscular injection of diclofenac sodium and lidocaine can cause many systemic adverse reactions and adverse reactions at the injection site in patients.Targeted preventive nursing measures should be taken to reduce the incidence of adverse reactions and ensure the medication safety of patients.
