^(18)F-AV45 PET/CT显像半定量分析在阿尔茨海默病早期诊断中的应用价值

摘要 目的:探讨^(18)F-AV45 PET/CT显像半定量分析在阿尔茨海默病早期诊断中的应用价值。方法:将2020年12月至2022年9月在郑州大学第一附属医院行^(18)F-AV45 PET/CT显像的12例健康志愿者、17例轻度认知障碍患者纳入研究。测定所有研究对象90个脑区的^(18)F-AV45标准摄取值比值(SUVR),应用ROC曲线分析各脑区SUVR诊断轻度认知障碍的最佳界值。结果:轻度认知障碍组90个脑区中有23个脑区的SUVR高于健康对照组(P均<0.05)。ROC曲线分析结果显示,这23个脑区中眶部额中回-左侧和颞中回-左侧的SUVR诊断轻度认知障碍的效能较高,曲线下面积(95%CI)分别为0.828(0.659~0.998)和0.824(0.653~0.994)。结论:眶部额中回-左侧和颞中回-左侧SUVR对诊断轻度认知障碍有较高的效能。^(18)F-AV45 PET有助于早期发现AD患者。 Aim:To investigate the application value of semi-quantitative analysis of^(18)F-Florbetapir(^(18)F-AV45)positron emission tomography/computed tomography(PET/CT)imaging in early diagnosis of Alzheimer′s disease(AD).Methods:Twelve health controls and 17 patients with mild cognitive impairment(MCI)who underwent^(18)F-AV45 PET/CT imaging in the First Affiliated Hospital of Zhengzhou University from December 2020 to September 2022 were included in this study.The^(18)F-AV45 standardized uptake value ratio(SUVR)of 90 brain regions of all study subjects was measured,and ROC curve analysis was used to determine the optimal threshold for diagnosing mild cognitive impairment using SUVR in each brain region.Results:SUVR in 23 out of 90 brain regions was significantly higher in MCI group than that in health control group(all P<0.05).The ROC curve analysis showed that the SUVR of the left orbital middle frontal gyrus and the left middle temporal gyrus had higher diagnostic efficiency in differentiating healthy people and MCI patients,the area under the curve(95%CI)was 0.828(0.659-0.998)and 0.824(0.653-0.994),respectively.Conclusion:SUVR of the left orbital middle frontal gyrus and the left middle temporal gyrus showed high efficacy in the diagnosis of MCI.^(18)F-AV45 PET is helpful for early detection of AD patients.
