中药穴位贴敷结合双心护理应用于冠状动脉粥样硬化性心脏病合并抑郁症的价值

Value of traditional Chinese medicine acupoint application combined with psycho-cardiology nursing in coronary atherosclerotic heart disease complicated with depression

目的探讨中药穴位贴敷结合双心护理应用于冠状动脉粥样硬化性心脏病(简称“冠心病”)合并抑郁症患者的价值。方法以2019年1月至2021年1月聊城市第四人民医院收治的150例冠心病合并抑郁症患者为研究对象,采用随机数字表法分为对照组(75例,应用常规护理)和观察组(75例,基于常规护理应用中药穴位贴敷结合双心护理)。对比2组的心绞痛发作情况、冠状动脉微循环功能、心功能、心理状态、心绞痛严重程度及预后。结果干预1个月和3个月后,观察组的心绞痛发作次数均少于同期对照组,心绞痛持续时间均短于同期对照组,冠状动脉微循环抵抗指数和左室射血分数均大于同期对照组,脑钠肽水平均低于同期对照组,汉密尔顿抑郁量表和西雅图心绞痛量表评分均低于对照组(P均<0.05);2组患者在干预1个月和3个月后的上述指标均较干预前改善(P均<0.05),干预3个月后的上述指标均较干预1个月后更佳(P均<0.05)。观察组的不良预后事件总发生率低于对照组(P<0.05)。结论中药穴位贴敷结合双心护理可有效改善冠心病合并抑郁症患者的冠状动脉微循环功能及心功能,减轻其心绞痛程度及抑郁情绪,改善预后。

Objective To explore the value of traditional Chinese medicine acupoint application combined with psycho-cardiology nursing in the treatment of coronary atherosclerotic heart disease(CHD)complicated with depression.Methods A total of 150 CHD patients with depression admitted to Liaocheng Fourth People's Hospital from January 2019 to January 2021 were included in the study.The patients were divided into a control group(75 cases for conventional nursing)and an observation group(75 cases for traditional Chinese medicine acupoint application combined with psycho-cardiology nursing based on the conventional nursing)by random number table method.The angina attacks,coronary microcirculatory function,cardiac function,mental state,angina severity,and prognosis were compared between the two groups.Results After 1 and 3 months of intervention,the observation group had fewer angina attacks,shorter angina duration,a higher index of microcirculatory resistance,a higher left ventricular ejection fraction,a lower brain natriuretic peptide level,and lower scores of the Hamilton Rating Scale for Depression and Seattle Angina Questionnaire when compared with the control group(all P<0.05).The above indicators in both groups were improved after 1 and 3 months of intervention when compared with those before intervention(all P<0.05),and the above indicators after 3 months of intervention were better those after 1 months of intervention(all P<0.05).The overall incidence of adverse prognostic events in the observation group was lower than that in the control group(P<0.05).Conclusion Traditional Chinese medicine acupoint application combined with psycho-cardiology nursing can effectively improve the coronary microcirculatory function and cardiac function in CHD patients with depression,alleviate the angina and depression,and improve the prognosis.
