摘要 目的:构建基于层次分析法(AHP)的逼近理想解排序法(TOPSIS)—灰色关联度法大型医用设备购置决策模型,以保障新建医院开诊中有效进行分阶段购置大型医用设备。方法:根据AHP确定购置决策模型的指标权重,通过TOPSIS得出各设备相对贴近度,通过灰色关联度法算出各设备相对关联度,根据专家组评估系数将相对贴近度和相对关联度整合为综合贴近度完成购置决策模型构建。选取医院临床检查和治疗常用的X射线计算机断层扫描(CT)、磁共振成像(MRI)、数字减影血管造影(DSA)、电子直线加速器(LA)、发射性型计算机断层扫描(ECT)和数字X射线摄影(DR)6种大型医用设备进行新建医院大型医用设备购置决策研究。结果:构建的大型医用设备购置决策模型计算的购置设备优先级排序为CT、MRI、DR、DSA、ECT和LA。根据资金情况和相关政策,新建医院第一批购置CT、MRI和DR三种设备,在新建医院开诊后半年内未出现临床检查和治疗挤兑等情况,医院整体运行平稳。结论:基于AHP的TOPSIS—灰色关联度法构建的大型医用设备购置决策模型应用于新建医院大型医用设备购置效果良好,能够有效满足新建医院平稳运行的需求。 Objective:To construct a decision-making model of large-scale medical equipment purchase with the method of technique for order preference by similarity to ideal solution(TOPSIS)-grey relational degree based on analytic hierarchy process(AHP),to solve the problem of phased purchase of large-scale medical equipment to ensure the opening of new hospitals.Methods:The index weight of the purchase decision model was determined according to AHP,the relative proximity of each equipment was obtained through TOPSIS,the relative correlation of each equipment was calculated by grey correlation degree method,and the relative proximity and relative correlation degree were integrated into the comprehensive proximity according to the evaluation coefficient of the expert group to complete the construction of the purchase decision model.X-ray computed tomography(CT),magnetic resonance imaging(MRI),digital subtraction angiography(DSA),electron linear accelerator(LA),emission computed tomography(ECT)and digital X-ray photography(DR)commonly used in hospital clinical examination and treatment were selected to study the large medical equipment purchase decision of new hospitals.Results:The constructed large medical equipment purchase decision-making model calculated the equipment purchase priority order was CT,MRI,DR,DSA,ECT and LA.According to the financial situation and relevant policies,CT,MRI and DR equipment were purchased for the first batch in the newly built hospital.Within half a year after the opening of the newly-built hospital,there was no clinical examination or treatment outage,and the overall operation of the hospital was stable.Conclusion:The decision-making model of large medical equipment purchase based on AHP TOPSIS-grey relational degree method has been applied to the purchase of large medical equipment in new hospitals.The research results were highly consistent with the actual purchase effect.The method was feasible and effective and can meet the requirements of the stable operation of new hospitals.
