中药灌肠治疗急性胰腺炎用药规律分析

Analysis of Medication Regularity of Chinese Herbal Enema for Acute Pancreatitis

目的:从数据挖掘角度探讨中药保留灌肠治疗急性胰腺炎的用药规律,为临床组方用药提供参考。方法:检索2017年1月-2022年4月知网、万方、维普数据库中中药灌肠治疗急性胰腺炎的相关文献并进行筛选,对合格文献进行药物数据分析,统计用药频率、性味归经、四气五味规律;运用IBM SPSS Statistics 26.0及SPSS Modeler 18软件进行关联规则分析和聚类分析。结果:共纳入文献106篇,有效方剂76个,涉及药物107味,累计用药频次717次。高频药物有大黄、芒硝、柴胡、厚朴、枳实、黄芩等;用药类型多为清热药、泻下药、活血化瘀药;药性多为寒、微寒、温;归经多为脾、胃、肝、大肠。关联规则分析得到高频药对大黄-芒硝、大黄-柴胡、大黄-厚朴等,聚类分析得出3种新处方,但疗效各有偏重。结论:通过统计分析可知,中药灌肠治疗急性胰腺炎多以清热解毒、泻下通腑、活血化瘀为主,这也为临床用药提供一定数据支持。

Objective:To explore the Regularity of Chinese herbal enema Treatment of Acute Pancreatitis based on data mining approaches.Methods:Search and filter the relevant literatures of Chinese herbal enema Treatment of Acute Pancreatitis in CNKI,WANFANG and VIP from January 2017 to April 2022,analyzed the qualified literature for drug data,and counted the statistics of frequency,taste and meridian entries,nature and flavor;SPSS Statistics 26.O and SPSS Modeler 18 software were used for analysising the association rules and cluster respectively.Results:106 Literatures were included,76 valid prescriptions were extracted,involving 107 kinds of traditional Chinese medicines,with a total medication frequency of 717.The high-frequency drugs were Rhubarb,Mirabilite,Chaihu,Magnolia of ficinalis,Fructus aurantii and Scutellariae radix;The types of medication used were mostly heat-clearing drugs,laxatives and blood-activating drugs;The drugs property weas mostly cold,slightly cold and warm;The channels were mostly attributed to spleen,stomach,liver and large intestine.The association rule analysis yielded high-frequency drug pairs of Rhubarb-Mirabilite,RhubarbChaihu,and Rhubarb-magnolia of ficinalis,which could be clustered to yield three new prescriptions,but each had a bias in therapeutic efficacy.Conclusion:The statistical analysis shows that Chinese herbal enema treatment for acute pancreatitis mainly focus on clearing heat and detoxifying,ventilating and laxing the internal organs,and promoting blood circulation to remove blood stasis,which also provides some data support for clinical use.
