经阴道三维超声检测卵巢卵泡数及血流动力学指标对多囊卵巢综合征导致不孕症的诊断价值

Diagnostic Value of Ovarian Follicle Number and Hemodynamic Indexes by Transvaginal Three-dimensional Ultrasound in Infertility Caused by Polycystic Ovary Syndrome

目的探讨经阴道三维超声检查卵巢卵泡数及血流动力学指标对多囊卵巢综合征(PCOS)导致不孕症的诊断价值。方法选取江西中医药大学第二附属医院2020年3月至2022年2月收治的53例PCOS导致不孕症患者,纳入试验组;另选取同期体检的53名健康者,纳入对照组。两组均经阴道三维超声检查卵巢卵泡数及血流动力学指标,比较两组卵巢卵泡数、血流动力学指标差异,分析经阴道三维超声检测卵巢卵泡数及血流动力学指标诊断PCOS导致不孕症的应用价值。结果试验组卵巢卵泡数多于对照组,卵巢收缩期峰值血流速度(PSV)、卵巢动脉舒张末期血流速度(EDV)均高于对照组,搏动指数(PI)、阻力指数(RI)低于对照组,差异有统计学意义(P<0.05);绘制受试者工作特征曲线(ROC)发现,卵巢卵泡数、PSV、EDV、PI、RI单独及联合检测诊断PCOS导致不孕症的AUC>0.70,均有一定诊断价值。结论经阴道三维超声检查卵巢卵泡数及血流动力学指标对PCOS导致不孕症具有较高的诊断价值。

Objective The diagnostic value of transvaginal three-dimensional ultrasound in detecting ovarian follicle count and hemodynamic indicators for infertility caused by polycystic ovary syndrome(PCOS)has been explored.Methods 53 patients with PCOS induced infertility admitted to the Second Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from March 2020 to February 2022 were selected and included in the experimental group;Another 53 healthy individuals who underwent physical examinations during the same period were selected and included in the control group;Both groups underwent vaginal three-dimensional ultrasound to detect the number of ovarian follicles and hemodynamic indicators.The differences in ovarian follicles and hemodynamic indicators between the two groups were compared,and the application value of vaginal three-dimensional ultrasound to detect ovarian follicles and hemodynamic indicators in diagnosing infertility caused by PCOS was analyzed.Results The number of ovarian follicles,maximum systolic velocity(PSV),and end diastolic velocity(EDV)of the ovarian artery in the experimental group were higher than those in the control group,while the pulse index(PI)and resistance index(RI)were lower than those in the control group(P<0.05);ROC curves revealed that the AUC of ovarian follicle count,PSV,EDV,PI,and RI,both individually and in combination,was greater than 0.70 for the diagnosis of infertility caused by PCOS,all of which have certain diagnostic value.Conclusion Transvaginal threedimensional ultrasound detection of ovarian follicle count and hemodynamic indicators has high diagnostic value for infertility caused by PCOS.
