基于“肺与大肠相表里”观察清燥救肺汤治疗肺热津伤型糖尿病性便秘的临床疗效

Clinical observation of Qingzao Jiufei Decoction on diabetic constipation with lung heat and fluid injury:Based on"the lung and large intestine being interior-exteriorly related"theory

目的基于“肺与大肠相表里”理论观察清燥救肺汤治疗肺热津伤型糖尿病性便秘患者的临床疗效。方法将60例糖尿病性便秘患者按照随机数字表法分为2组,对照组30例予双歧杆菌三联活菌胶囊治疗,治疗组30例在对照组治疗基础上加用清燥救肺汤治疗。2组均治疗4周,比较2组治疗前后大便性状评分、每周大便次数、空腹血糖(FPG)、餐后2 h血糖(2 hPG)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、同型半胱氨酸(Hcy)变化,并统计2组疗效。结果治疗组总有效率80.0%(24/30),对照组总有效率63.3%(19/30),治疗组临床疗效优于对照组(P<0.05)。2组治疗后大便性状评分均较本组治疗前升高(P<0.05),且治疗组治疗后高于对照组(P<0.05)。2组治疗后每周大便次数均较本组治疗前增加(P<0.05),且治疗组治疗后高于对照组(P<0.05)。治疗组治疗后FPG、2 hPG水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。治疗组治疗后TC、TG、LDL-C水平均较本组治疗前降低(P<0.05),且治疗组治疗后TC、TG、LDL-C水平均低于对照组(P<0.05)。治疗组治疗后Hcy水平较本组治疗前降低(P<0.05),且治疗组低于对照组(P<0.05)。结论清燥救肺汤对肺热津伤型糖尿病性便秘患者疗效显著,可以降低患者血糖,调节血脂,改善患者的大便性状及增加排便次数,并能降低Hcy水平。

Objective To observe the clinical efficacy of Qingzao Jiufei Decoction in the treatment of diabetic constipation patients with lung heat and fluid injury based on the theory of based on"the lung and the large intestine being interior-exteriorly related".Methods 60 patients with diabetic constipation were randomlygrouped.On the basis of bifidobacterium triple viable capsule(the control group,n=30),the treatment group were treated with Qingzao Jiufei decoctionfor 4 weeks.The fasting blood glucose(FPG),2-h postprandial blood glucose(2hPG),stool trait score,weekly stool frequency,triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)and homocysteine(Hcy)were included as comparisons before and after treatment between the two groups,and the efficacy of the two groups was counted.Results After treatment,the total effective rate of the treatment group was superior to the control group(80.0%[24/30]vs 63.3%[19/30],P<0.05).Stool traits scores and weekly stool frequency in the two groups were increased higher than those before treatment(P<0.05),and the treatment group was more common than the control group(all P<0.05).FPG,2hPG,TC,TG and LDL-C and Hcy in the treatment group were decreased than those before treatment(P<0.05),which were lower in the treatment group than in the control group(all P<0.05).There was no significant change in HDL-C level before and after treatment in the two groups.Conclusion For patients diabetic constipation(lung heat and fluid injury),Qingzao Jiufei Decoction has a significant effect.It can reduce blood glucose,regulate blood lipids,improve stool traits and increase stool frequency,and reduce Hcy levels.
