头针结合悬吊训练对脑梗死后平衡障碍患者运动功能的影响 被引量:1

摘要 目的观察头针结合悬吊训练应用于脑梗死后平衡障碍患者的康复效果。方法将94例脑梗死后平衡障碍患者按照随机数字表法分为2组,对照组47例在脑梗死常规治疗基础上进行悬吊训练,治疗组47例在脑梗死常规治疗基础上予以头针治疗,留针期间进行悬吊训练(方法同对照组)。2组均治疗4周。比较2组治疗前后Berg平衡量表(BBS)评分、简式Fugl-Meyer运动功能(FMA)量表评分、本体感觉指标平均轨迹误差(ATE)和平均测试执行时间(TTE),应用起立行走计时测验(TUGT)评价2组跌倒风险,10 m最大步行速度(MWS)评价2组运动能力,并比较2组临床疗效。结果治疗组总有效率95.74%(45/47),对照组总有效率85.11%(40/47),治疗组临床疗效优于对照组(P<0.05)。2组治疗后BBS、FMA评分均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。治疗组治疗后健侧、患侧ATE、TTE均较本组治疗前降低(P<0.05),且低于对照组治疗后(P<0.05),对照组治疗前后差异均无统计学意义(P>0.05)。2组治疗后TUGT均较本组治疗前降低(P<0.05),MWS均升高(P<0.05),且治疗组治疗后TUGT低于对照组(P<0.05),MWS高于对照组(P<0.05)。结论头针结合悬吊训练有利于改善患者本体感觉,增强平衡能力及下肢运动能力,应用于脑梗死后平衡障碍的康复治疗效果显著。 Objective To explore the rehabilitation effect of scalp acupuncture combined with suspension training for balance disorder patients with cerebral infarction.Methods Totally 94 patients with balance disorder due to cerebral infarction were randomized 1∶1 to receive suspension training+routine treatment(control group)or to scalp-acupuncture+suspension training+routine treatment(the treatment group)for 4 weeks.The key observation were the scores of Berg Balance Scale(BBS),simple Fugl-Meyer motor function(FMA),and the average trajectory error(ATE)and average test time execution(TTE).The fall risk was evaluated by timed up and go test(TUGT),and the motor ability was evaluated by 10m maximum walking speed(MWS),and the clinical effects of the two groups were compared.Results After treatment,the total effective rate in the treatment group was better than that in the control group(95.74%[45/47]vs 85.11%[40/47],P<0.05).The scores of BBS and FMA in the two groups were higher than those before treatment(P<0.05),and which in the treatment group were higher than in the control group(P<0.05).ATE and TTE in the treatment group were lower than those before treatment(P<0.05),and which were lower than in the control group(P<0.05).There was no significant difference in ATE and TTE in the control group before and after treatment(P>0.05).TUGT in both groups was lower than that before treatment(P<0.05),and MWS was higher(P<0.05),and the treatment group was superior to control group for TUGT and MWS(all P<0.05).Conclusion Scalp-acupuncture combined with suspension training is helpful to improve patients'proprioception,enhance balance ability and lower limb motor ability.It has a significant effect on the rehabilitation of balance disorder after cerebral infarction.
