中药熏洗结合穴位贴敷在腹膜透析尿毒症皮肤瘙痒患者临床护理中的应用

Application of Chinese medicine fumigation combined with acupoint application in clinical nursing of patients with pruritus uremia on peritoneal dialysis

目的分析中药熏洗结合穴位贴敷在腹膜透析尿毒症皮肤瘙痒患者临床护理中的应用效果。方法选取2021年11月至2022年7月于九江市中医医院80例腹膜透析尿毒症皮肤瘙痒患者为研究对象,按照随机数字表法分为干预组和对照组,每组40例。对照组常规护理,干预组在对照组基础上予以中药熏洗结合穴位贴敷干预。比较两组干预前后皮肤瘙痒症状改善情况、生活质量评估及生化指标水平。结果干预后干预组患者瘙痒分布部位、严重程度、频率、对睡眠影响及总分均低于对照组,差异有统计学意义(P<0.05);干预后干预组患者皮肤病生活质量指数(DLQI)低于对照组,差异有统计学意义(P<0.05);干预后干预组患者血清中钙含量低于对照组,磷、血红蛋白含量高于对照组,差异有统计学意义(P<0.05)。结论中药熏洗结合穴位贴敷可有效改善腹膜透析尿毒症皮肤瘙痒患者的瘙痒症状,提高生活质量,并改善钙磷代谢及血红蛋白水平。

Objective To analyze the application of Chinese medicine fumigation combined with acupoint application in clinical nursing of patients with pruritus uremia on peritoneal dialysis.Methods A total of 80 patients with pruritus and uremia undergoing peritoneal dialysis in Jiujiang Hospital of Traditional Chinese Medicine were enrolled between November 2021 and July 2022.They were divided into intervention group and control group by random number table method,with 40 cases in each group.The control group received routine nursing,and the intervention group received traditional Chinese medicine fumigation combined with acupoint application on the basis of the control group.The improvement of pruritus symptoms,quality of life assessment and biochemical indexes were compared between the two groups before and after intervention.Results After intervention,the pruritus distribution site,severity,frequency,effect on sleep and total score in the intervention group were lower than those in the control group,with statistical significances(P<0.05).After intervention,the skin disease life quality index(DLQI)in the intervention group was lower than that in the control group,and the difference was statistically significant(P<0.05).After intervention,the serum calcium content in the intervention group was lower than that in the control group,while the contents of phosphorus and hemoglobin were higher than that in the control group,with statistical significances(P<0.05).Conclusion Traditional Chinese medicine fumigation combined with acupoint application can effectively improve pruritus symptoms,improve quality of life,calcium and phosphorus metabolism and hemoglobin level in patients with uremia on peritoneal dialysis.
