强直性脊柱炎患者近端肾小管损伤相关生物学标志物

Analysis of Biomarkers Related to Proximal Renal Tubular Injury in Ankylosing Spondylitis

摘要 目的探讨强直性脊柱炎患者近端肾小管损伤的尿生物学标志物。方法选取2019年1月—2020年12月寿光市人民医院收治的70例强直性脊柱炎患者,按照尿白蛋白/肌酐比值将其分为蛋白尿组40例与无蛋白尿组30例,另外选取同期70例健康体检者为对照组。比较三组间尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平。强直性脊柱炎患者进行Bath强直性脊柱炎疾病活动指数评分,并分析该评分与尿液检测指标的关系。结果三组间尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平,差异有统计学意义(P<0.05)。相关性分析显示,Bath强直性脊柱炎疾病活动指数评分与尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平呈正相关。结论有微量白蛋白尿的强直性脊柱炎患者肾小管损伤程度更严重,尿液β2-微球蛋白、肾损伤分子-1、中性粒细胞明胶酶相关脂质运载蛋白、白介素-18、N-乙酰-β-氨基葡萄糖苷酶、视黄醇结合蛋白水平,可作为评估强直性脊柱炎患者早期近端肾小管损伤的指标。 Objective To explore the urinary biomarkers of proximal renal tubular injury in patients with ankylosing spondylitis.MethodsA total of 70 patients with ankylosing spondylitis admitted to Shouguang People's Hospital from January 2019 to December 2020 were selected,and were divided into proteinuria group(40 cases)and non-proteineuria group(30 cases)according to the ratio of urinary albumin/creatinine.In addition,70 health examination subjects were selected in the same period as the control group.The urineβ2-microglobulin(β2-MG),kidney injury molecule-1(KIM-1),neutrophil gelatinase associated lipid carrier protein(NGAL),interleukin-18(IL-18),N-acetyl-β-glucosaminidase(NAG)and retinol-binding protein(RBP)levels were compared among three groups.The patients with ankylosing spondylitis were scored with the Bath ankylosing spondylitis disease activity index(BAS-DAI),and the relationship between the score and the urine test index was analyzed.Results The urine levels ofβ-MG,KIM-1,NGAL,IL-18,NAG and RBP among three groups were significantly different(P<0.05).Correlation analysis showed that the scores of BAS-DAI and urine levels ofβ2-MG,KIM-1,NGAL,IL-18,NAG and RBP were positively correlated.Conclusion In ankylosing spondylitis patients with microalbuminuria,the degree of renal tubular injury is more severe,and urine levels ofβ2-MG,KIM-1,NGAL,IL-18,NAG and RBP can be used as an indicator to evaluate early proximal renal tubular injury in patients with ankylosing spondylitis.
