宫腔镜和经阴道超声检查对绝经后子宫内膜增厚患者的诊断效果

Diagnostic Effect of Hysteroscopy and Transvaginal Ultrasound on Postmenopausal Endometrial Thickening

目的比较分析宫腔镜和经阴道超声检查对绝经后子宫内膜增厚患者的诊断效果。方法选取2020年1月—2022年11月在闽清县总医院就诊的56例疑似绝经后子宫内膜增厚患者,全部患者均行宫腔镜、经阴道超声及病理学检查,并以病理学检查作为金标准,比较分析宫腔镜、经阴道超声及病理学检查的诊断一致性,以及宫腔镜和经阴道超声检查对子宫内膜增厚的诊断效果。结果宫腔镜检查与病理学检查结果的一致性较好,经阴道超声检查与病理学检查结果的一致性一般,宫腔镜与经阴道超声检查结果的一致性一般。宫腔镜检查的诊断符合率较经阴道超声检查高,差异有统计学意义(P<0.05)。宫腔镜检查对子宫内膜息肉的检出率高于经阴道超声检查,差异有统计学意义(P<0.05)。宫腔镜检查与经阴道超声检查对内膜增生、黏膜肌瘤、内膜癌及正常的检出率,差异均无统计学意义(P>0.05)。结论宫腔镜检查对绝经后子宫内膜增厚患者的诊断符合率高于经阴道超声检查,其诊断结果与病理结果符合程度较高,值得在临床上推广应用。

Objective To compare and analyze the diagnostic effects of hysteroscopy and transvaginal ultrasound on postmenopausal patients with endometrial thickening.Methods A total of 56 patients with suspected postmenopausal endometrial thickening who were treated in Minqing County General Hospital from January 2020 to November 2022 were selected.All patients were examined by hysteroscopy,transvaginal ultrasound and pathology,and pathological examination was taken as the gold standard.The diagnostic consistency of hysteroscopy,transvaginal ultrasound and pathology,and the diagnostic effect of hysteroscopy and transvaginal ultrasound on endometrial thickening were compared and analyzed.Results The results of hysteroscopy and pathology are in good consistency,the results of transvaginal ultrasound and pathology are in general consistency,and the results of hysteroscopy and transvaginal ultrasound are in general consistency.The diagnostic accuracy of hysteroscopy was significantly higher than that of transvaginal ultrasound(P<0.05).The detection rate of endometrial polyps by hysteroscopy was significantly higher than that by transvaginal ultrasound(P<0.05).There was no significant difference between hysteroscopy and transvaginal ultrasound in the detection rate of endometrial hyperplasia,mucosal myoma,endometrial cancer and normal(P>0.05).Conclusion The diagnostic accuracy of hysteroscopy in postmenopausal patients with endometrial thickening is higher than that of transvaginal ultrasound,and the diagnostic results are consistent with the pathological results,which is worthy of clinical application.
