国医大师肖承悰运用胡芦巴丸加减治疗子宫腺肌病致痛经的临床经验 被引量:1

摘要 子宫腺肌病是由子宫内膜腺体及间质侵入子宫肌层引起的、以进行性加重的痛经为主要临床表现的一种妇科常见病、疑难病。根据其临床表现归属于“痛经”的范畴。国医大师肖承悰教授认为子宫腺肌病致痛经的主要病机为肾阳亏虚、寒凝血瘀,肾阳亏虚是其发病基础,寒凝血瘀是其病理特征。辨治应温通并重、标本兼顾。从温阳祛寒、行气止痛,化瘀通脉、分期论治,扶助正气、调和气血等方面介绍肖教授运用胡芦巴丸加减治疗子宫腺肌病致痛经的经验。结合临床病案分析肖教授的治疗方法和用药思路。 Adenomyosis,a common disease in gynecology which is hard to cure,is caused by the invasion of endometrial glands and stroma into the myometrium,with progressive and aggravated dysmenorrhea as its major clinical manifestation.It falls into the category of“dysmenorrhea”in traditional Chinese medicine according to its clinical manifestations.Master of traditional Chinese medicine Xiao Chengzong believes that the main pathogenesis of dysmenorrhea induced by adenomyosis is kidney Yang deficiency,cold coagulation and blood stasis.Kidney Yang deficiency is considered to be the basis of its pathogenesis,while cold coagulation and blood stasis is the pathological feature.In pattern differentiation and treatment,equal attention should be paid to both warming and unblocking,and to both symptoms and root causes.Professor Xiao’s experience with modified Huluba Pill in treating dysmenorrhea caused by adenomyosis is presented from the following aspects of warming Yang to dissipate cold,moving Qi to relieve pain,transforming blood stasis and unblocking meridians stage-based treatment,strengthening and supporting healthy Qi,and harmonizing Qi and blood.Professor Xiao’s ideas of treatment and medication are presented in combination with clinical medical records.
