ConView麻醉深度监测仪的麻醉深度指数在全凭静脉麻醉中的应用效果 被引量:1

摘要 目的探讨ConView麻醉深度监测仪的麻醉深度指数(CSI)在全凭静脉麻醉中的应用效果。方法将118例在全凭静脉麻醉下进行手术治疗的中耳炎患者分为A组(58例)与B组(60例)。A组在全凭静脉麻醉过程中采用ConView麻醉深度监测仪的CSI进行麻醉深度监测,B组根据血压、心率进行麻醉深度监测。比较两组患者不同时间点的平均动脉压(MAP)、心率及血糖水平,比较两组患者术中依托咪酯及瑞芬太尼用量、苏醒时间、拔管时间、术中知晓发生率,以及苏醒期躁动评分、术后恶心呕吐评分、术后疼痛评分。结果两组患者的MAP、心率、血糖水平比较,差异均无统计学意义(均P>0.05)。两组患者均无术中知晓发生。A组患者术中依托咪酯用量少于B组,苏醒时间、拔管时间均短于B组(均P<0.05),而两组患者的术中瑞芬太尼用量、苏醒期躁动评分、术后恶心呕吐评分及疼痛评分比较,差异均无统计学意义(均P>0.05)。结论在全凭静脉麻醉过程中采用ConView麻醉深度监测仪的CSI进行麻醉深度监测,能够在一定程度上指导医生用药,减少术中依托咪酯用量,缩短患者苏醒时间及拔管时间,有效避免术中知晓的发生,且对血流动力学无明显影响,安全性较高。 Objective To explore the application effect of cerebral state index(CSI)of ConView anesthesia depth monitor in total intravenous anesthesia.Methods A total of 118 patients with otitis media undergoing surgical therapy via total intravenous anesthesia were assigned to group A(58 cases)or group B(60 cases).Group A received CSI of ConView anesthesia depth monitor to perform anesthesia depth monitoring during total intravenous anesthesia,and group B received anesthesia depth monitoring according to blood pressure and heart rate.The mean arterial pressure(MAP),heart rate,and blood glucose level of patients at different time points were compared between the two groups.The intraoperative dosages of etomidate and remifentanil,recovery time,extubation time,incidence rate of intraoperative awareness,agitation score during recovery,postoperative nausea and vomiting score,and postoperative pain score of patients were compared between the two groups.Results No statistically significant difference in MAP,heat rate,and blood glucose level of patients was found between the two groups(all P>0.05).The intraoperative awareness of patients in both groups did not occurred.The intraoperative dosage of etomidate in group A was less than that in group B,and recovery time,extubation time in group A were shorter than those in group B(all P<0.05),whereas no statistically significant difference in the intraoperative dosage of remifentanil,and scores of agitation during recovery,postoperative nausea and vomiting,and postoperative pain of patients was found between the two groups(all P>0.05).Conclusion Employing CSI of ConView anesthesia depth monitor to perform anesthesia depth monitoring during total intravenous anesthesia can guide the medication for doctors to a certain extent,reduce the intraoperative dosage of etomidate,shorten patients′recovery time and extubation time,effectively avoid the occurrence of intraoperative awareness,and it presents no significant effect on hemodynamics,exerting a relatively high safety.
