帕博利珠单抗联合CapeOx方案治疗Her2阳性晚期胃癌的近远期效果

The short-term and long-term effects of pembrolizumab combined with CapeOx regimen in the treatment of Her2 positive advanced gastric cancer

目的探讨帕博利珠单抗联合CapeOx方案治疗人表皮生长因子受体-2(Her2)阳性晚期胃癌的近远期效果。方法按照治疗方式的不同将56例Her2阳性晚期胃癌患者分为对照组(n=28,CapeOx方案)和观察组(n=28,帕博利珠单抗+CapeOx方案)。比较两组的治疗效果。结果观察组的近期治疗总缓解率高于对照组(P<0.05)。治疗后,观察组的癌胚抗原(CEA)、糖类抗原-125(CA-125)、糖类抗原-199(CA-199)水平及白细胞分化抗原8阳性(CD8^(+))低于对照组,白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))高于对照组(P<0.05)。两组的毒副反应总发生率无显著差异(P>0.05)。治疗后,观察组的癌症患者生命质量测定量表(FACT-G)评分高于对照组(P<0.05);观察组的无进展生存期和总生存期均长于对照组(P<0.05)。结论帕博利珠单抗联合CapeOx方案治疗Her2阳性晚期胃癌的近远期效果显著,可降低肿瘤标记物水平,提高患者的免疫功能及生活质量,且不会增加毒副反应。

Objective To investigate the short-term and long-term effects of pembrolizumab combined with CapeOx regimen in the treatment of human epidermal growth factor receptor 2(Her2)positive advanced gastric cancer.Methods According to different treatment methods,56 Her2 positive advanced gastric cancer patients were divided into control group(n=28,CapeOx regimen)and observation group(n=28,pembrolizumab+CapeOx regimen).The therapeutic effects of the two groups were compared.Results The total remission rate of short-term treatment of the observation group was higher than that of the control group(P<0.05).After treatment,the levels of carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA-125),carbohydrate antigen 199(CA-199)and cluster of differentiation 8 positive(CD8+)in the observation group were lower than those in the control group,and the cluster of differentiation 3 positive(CD3^(+))and cluster of differentiation 4 positive(CD4^(+))were higher than those in the control group(P<0.05).There was no significant difference in the total incidence of toxic and side effects between the two groups(P>0.05).After treatment,the Functional Assessment of Cancer Therapy-Generic Scale(FACT-G)score of the observation group was higher than that of the control group(P<0.05);the progression-free survival and overall survival of the observation group were longer than those of the control group(P<0.05).Conclusion The pembrolizumab combined with CapeOx regimen in the treatment of Her2 positive advanced gastric cancer has significant short-term and long-term effects.It can reduce the levels of tumor markers and improve the immune function and quality of life of patients,and doesn't increase toxic and side effects.
