摘要
目的探讨甲状腺功能正常人群甲状腺激素水平与代谢相关脂肪性肝病(MAFLD)的相关性。方法选取2019年1月至12月就诊于苏州市相城区太平街道社区卫生服务中心的120例健康体检者作为研究对象,根据是否患有MAFLD将其分为正常组(n=79)和MAFLD组(n=41)。比较两组的一般资料;分析甲状腺激素水平与代谢相关指标的关系;用多因素Logistic回归分析探讨影响MAFLD发生的危险因素。结果研究对象的平均年龄(46.38±9.69)岁,其中41例符合MAFLD诊断标准,患病率为34.17%。MAFLD组的促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT_(3))水平均高于正常组,差异有统计学意义(P<0.05)。Pearson秩相关分析结果显示:TSH与腰围(WC)、体重指数(BMI)、空腹血糖(FPG)、总胆固醇(TC)、天冬氨酸氨基转移酶(AST)、γ-谷氨酰转肽酶(GGT)呈正相关,与高密度脂蛋白胆固醇(HDL-C)呈负相关(P<0.05);FT_(3)与WC、BMI、FPG、TG、丙氨酸氨基转移酶(ALT)、AST、GGT呈正相关,与HDL-C呈负相关(P<0.05);游离甲状腺素(FT_(4))与指标间无相关性(P>0.05)。多因素Logistic回归分析显示,TSH、FT_(3)水平是影响MAFLD发生的独立危险因素(P<0.05)。结论甲状腺功能正常人群中TSH、FT_(3)水平是影响MAFLD发生的独立危险因素。
Objective To investigate the correlation between thyroid hormone levels and metabolic associated fatty liver disease(MAFLD)in people with normal thyroid function.Methods A total of 120 healthy subjects who visited Taiping Street Community Health Service Center in Xiangcheng District of Suzhou City from January to December 2019 were selected as the research objects,and the subjects were divided into normal group(n=79)and MAFLD group(n=41)according to whether they had MAFLD.The general data of the two groups were compared;the relationship between thyroid hormone levels and metabolic related indicators was analyzed;multivariate Logistic regression analysis was used to explore the risk factors affecting the occurrence of MAFLD.Results The average age of the subjects was(46.38±9.69)years old,of which 41 cases met the diagnostic criteria of MAFLD,and the prevalence rate was 34.17%.The levels of thyroid stimulating hormone(TSH)and free triiodothyronine(FT_(3))in the MAFLD group were higher than those in the normal group,and the differences were statistically significant(P<0.05).Pearson rank correlation analysis showed that TSH was positively correlated with waistline(WC),body mass index(BMI),fasting plasma glucose(FPG),total cholesterol(TC),aspartate aminotransferase(AST)andγ-glutamyl transpeptidase(GGT),and negatively correlated with high density lipoprotein-cholesterol(HDL-C)(P<0.05);FT_(3) was positively correlated with WC,BMI,FPG,TG,alanine aminotransferase(ALT),AST and GGT,and negatively correlated with HDL-C(P<0.05);there was no correlation between free thyroxine(FT_(4))and indicators(P>0.05).Multivariate Logistic regression analysis showed that TSH and FT_(3) levels were independent risk factors for MAFLD(P<0.05).Conclusion In people with normal thyroid function,TSH and FT_(3) levels were independent risk factor for MAFLD.
