2017—2021年湖州市流感样聚集性病例病原监测及腺病毒基因特征分析

摘要 目的 了解2017—2021年湖州市流感样聚集性病例主要呼吸道病毒病原特征,并研究腺病毒的基因型别,为呼吸道病毒的防控,特别是腺病毒的防控提供实验室依据。方法 收集2017—2021年湖州市流感样聚集性疫情病例的鼻咽拭子样本,使用多重聚合酶链反应(polymerase chain reaction,PCR)方法进行呼吸道病原体的检测,并对腺病毒阳性样本扩增Hexon基因测序分析。结果 共检测754例样本,检出阳性364例,阳性率48.28%。乙型流感病毒检出率最高,占40.11%(146/364);其次为甲型流感病毒占24.18%(88/364)、腺病毒占14.84%(54/364)、鼻病毒占13.74%(50/364)、偏肺病毒占4.67%(17/364)、呼吸道合胞病毒占1.92%(7/364)、间质肺病毒占0.55%(2/364)。51例腺病毒Hexon基因测序分析显示,基因型分属B组和C组,其中B3型27例,B7型6例,B21型3例,C1型9例,C2型6例。结论 湖州市流感样聚集性疫情病原以流感病毒为主,腺病毒次之,腺病毒感染以B3型为主。 Objective To understand the pathogenic characteristics of major respiratory viruses in cluster cases with influenza-like illness(ILI)in Huzhou from 2017 to 2021,and to study the genetic characteristics of adenovirus to provide laboratory evidence for the prevention and control of respiratory viruses,especially adenovirus.Methods Throat swab samples were collected from cluster cases with ILI in Huzhou city during 2017 and 2021.The respiratory pathogens were detected with multiplex PCR.Hexon gene of adenovirus was amplified and sequenced among adenovirus positive samples.Results A total of 754 specimens were tested and 364 were positive with a positive detection rate of 48.28%.The detection rate of influenza B virus was the highest,accounting for 40.11%(146/364);followed by influenza A virus,accounting for 24.18%(88/364).The positive rates of adenovirus,rhinovirus,human metapneumovirus,respiratory syncytial virus and interstitial pulmonary virus were 14.84%(54/364),13.74%(50/364),4.67%(17/364),1.92%(7/364),0.55%(2/364),respectively.The hexon gene sequence analysis of 51 adenovirus positive samples showed that 27 were HAdV-B3,6 were HAdV-B7,3 were HAdV-B21,9 were HAdV-C1and 6 were HAdV-C2.Conclusions Influenza virus is the leading pathogen responsible for a cluster outbreak of ILI in Huzhou,followed by adenoviruses,which mostly belongs to HAdV-B3.
