探讨定量DCE联合T1 mapping鉴别乳腺癌淋巴结转移的效能

Quantitative DCE Combined with T1Mapping to Identify Lymph Node Metastases in Breast Cancer

目的:探讨定量磁共振动态增强(DCE-MRI)联合纵向弛豫时间定量成像(T1 mapping)鉴别乳腺癌淋巴结转移的效能。方法:回顾性分析2021年12月—2022年9月在我院经病理证实为乳腺癌且行DCE-MRI及T1 mapping检查的70例患者的临床资料,评估定量DCE联合T1 mapping参数鉴别乳腺癌淋巴结转移的价值。结果:单因素及多因素logistic分析结果显示EER、Tpeak、ADC、T1post、△T1%是评估乳腺癌淋巴结转移的独立参数(P<0.05)。ROC曲线结果显示EER、Tpeak、ADC、T1post、△T1%单独或联合评估乳腺癌淋巴结转移的AUC均>0.70,且联合评估效能更好。结论:DCE-MRI联合T1 mapping检查的EER、Tpeak、ADC、T1post、△T1%参数具有较高的乳腺癌淋巴结转移评估效能,且上述参数联合评估的效能最佳。

Objective:To investigate the efficacy of dynamic quantitative magnetic resonance enhancement(DCE-MRI)combined with quantitative longitudinal relaxation time imaging(T1 mapping)in the identification of lymph node metastases from breast cancer.Methods:From December 2021 to September 2022,70 female patients who were pathologically confirmed with breast cancer in our hospital and underwent DCE-MRI and T1 mapping examination were selected as research objects.To evaluate the value of quantitative DCE combined with T1 mapping parameters in the identification of lymph node metastasis in breast cancer.Results:There were differences in EER,Slope,T1pre,T1post,Simax,Tpeak,ADC and△T1%between the two groups(P<0.05).Multivariate logistic analysis showed that EER,Tpeak,ADC,T1post and△T1%were independent parameters for evaluating lymph node metastasis of breast cancer(P<0.05).The results of ROC curve showed that the AUC of EER,Tpeak,ADC,T1post and△T1%were all higher than 0.70,and the combined evaluation was more effective.Conclusions:The EER,Tpeak,ADC,T1post and△T1%parameters of DCE-MRI combined with T1 mapping have high efficacy in the evaluation of breast cancer lymph node metastasis,and the efficacy of the above parameters combined is the best.
