牙周微创手术联合不同再生生物材料治疗牙周骨缺损的临床效果研究

Clinical Effect of Periodontal Minimally Invasive Surgery Combined with Different Regenerative Biomaterials in the Treatment of Periodontal Bone Defects

目的:研究牙周微创手术联合不同再生生物材料治疗牙周骨缺损的临床效果。方法:随机数字表法将2021年6月—2022年6月在九江学院附属口腔医院就诊的88例牙周骨缺损患者分为观察组和对照组,各44例,两组均行牙周微创手术,观察组使用富血小板纤维蛋白膜(PRF),对照组使用海奥胶原膜,比较两组治疗前后牙周指数、缺损牙缺损高度及骨密度、美学效果、口腔健康。结果:治疗6个月,两组牙周指数牙周袋深度(PD)、附着丧失(AL)、根分叉水平探入深度(HPD)、牙龈指数(GI)、龈沟出血指数(SBI)降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组缺损牙缺损高度均降低,且观察组低于对照组,缺损牙骨密度升高,且观察组高于对照组,差异有统计学意义(P<0.05);两组粉色美学指数评分(PES)、白色美学指数评分(WES)均升高,且观察组高于对照组,差异有统计学意义(P<0.05);两组口腔健康影响程度量表(OHIP-14)中口腔疼痛与不适、独立能力、心理不适、身体机能评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:牙周微创手术联合PRF治疗牙周骨缺损患者能改善牙周指数和口腔健康,降低缺损牙缺损高度,增加缺损牙骨密度,提升美学效果。

Objective:To study the clinical effect of periodontal minimally invasive surgery combined with different regenerative biomaterials in the treatment of periodontal bone defects.Methods:88 patients with periodontal bone defects who were treated in the Affiliated Stomatological Hospital of Jiujiang University from June 2021 to June 2022 were divided into the observation group and the control group,with 44 patients in each group.Both groups underwent periodontal minimally invasive surgery.The observation group used platelet-rich fibrin membrane(PRF),and the control group used collagen membrane.The periodontal index,defect height,bone density,aesthetic effect,and oral health of the two groups were compared before and after treatment.Results:After 6 months of treatment,the periodontal index,periodontal pocket depth(PD),attachment loss(AL),horizontal probing depth(HPD),gingival index(GI),gingival sulcus bleeding index(SBI)of the two groups decreased,and the observation group was significantly lower than the control group(P<0.05);The height of defect teeth in the two groups decreased,and the bone density of defect teeth in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05);The pink esthetic score(PES)and White esthetic score(WES)of the two groups were both higher,and the observation group was higher than the control group,the difference was statistically significant(P<0.05);The scores of oral pain and discomfort,independent ability,psychological discomfort and physical function in the oral health impact scale(OHIP-14)of the two groups were lower,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Periodontal minimally invasive surgery combined with PRF can improve the periodontal index and oral health,reduce the height of defect teeth,increase the bone density of defect teeth,and improve the aesthetic effect.
