基于验案数据挖掘探究名中医连建伟治疗胃食管反流病的用药规律

Exploring the Medication Rules of Professor LIAN Jianwei in the Treatment of Gastroesophageal Reflux Disease Based on Data Mining

[目的]运用数据挖掘技术对连建伟教授治疗胃食管反流病验案的用药规律进行分析。[方法]运用中医传承辅助系统(V2.5),对2017年10月至2020年10月在连建伟名中医工作室就诊的胃食管反流病患者的用药进行频次统计、药物关联规则、复杂系统熵聚类等分析。[结果]共纳入胃食管反流病患者141例,男性61例、女性80例,50~59岁人数最多,病案274则。高频药物(频次≥30)共32种,核心药物由柴胡、郁金、丹参、陈皮、茯苓、甘草组成,是逍遥散加减方。药物四气以温、平、寒为主,五味以苦、甘、辛居多,归经多归脾、肝、胃经。熵聚类分析共提取出核心组合10组,新方5个。[结论]连教授治疗胃食管反流病时,常取逍遥散加减以疏肝健脾,用药寒温并用,补而不滞,同时注重随证治之,多随证加入清热、活血、消食、祛湿之法。

[Objective]Using data mining technology to analyze the medication rules of Professor LIAN Jianwei in the treatment of gastroesophageal reflux disease.[Methods]Using Traditional Chinese Medicine Inheritance Auxiliary System(V2.5),the drug frequency statistics,drug association rules,and complex system entropy clustering of gastroesophageal reflux patients treated at LIAN Jianwei's Famous Doctor Studio from October 2017 to October 2020 were analyzed.[Results]A total of 141 patients with gastroesophageal reflux disease were included,with 61 males and 80 females,most people aged 50~59,with 274 medical records.There were 32 kinds of high-frequency drugs(frequency≥30),and the commonly used drug pairs were composed of Bupleurum,Curcuma Radix,Salvia miltiorrhiza,dried tangerine peel,Poria,Licorice,and they were the addition and subtraction of Xiaoyao Powder.The four Qi of the drug were mainly warm,flat and cold,and the five flavors were mostly bitter,sweet and acrid,and most of the meridians belonged to the spleen,liver and stomach meridians.A total of 10 core combinations were extracted from the entropy cluster analysis,including 5 new ones.[Conclusion]In the treatment of gastroesophageal reflux disease,Professor LIAN often uses Xiaoyao Powder to soothe the liver and invigorate the spleen,and used both cold and warm drugs,tonic but not stagnant.At the same time,he pays attention to the treatment according to the syndrome,and adds the methods of clearing heat,promoting blood circulation,helping digestion and clearing dampness according to the syndrome.
