419例口腔癌患者住院费用分析

Analysis of Hospitalization Expenses in 419 Patients with Oral Cancer

目的分析口腔癌患者的住院情况和住院费用的影响因素,为合理控制医疗费用,降低患者的经济负担提供参考依据。方法选取某院2017年1月1日-2021年12月31日首次就诊且第一诊断为口腔癌的住院病案首页资料,采用描述分析、新灰色关联分析、结构变动分析和多元线性回归等统计方法对口腔癌患者的基本情况和住院费用进行分析。结果419例患者平均住院总费用为15008.20元,诊断类占比最多(27.52%),其次为西药类(25.65%),治疗类(21.69%)。年均结构变动度为13.90%,影响结构变动的主要类别是诊断类、耗材类和西药类,累计贡献率为81.57%,诊断类为正变动,耗材和西药类为负变动。口腔癌患者住院费用各类别的关联系数从大到小前3位依次为诊断类、西药类、治疗类。影响住院费用的因素从大到小依次为手术范围、住院天数、出院年份、出院诊断个数。结论口腔癌患者住院费用结构逐渐趋于合理,但诊断类费用仍是重点调整对象;可以通过合理控制住院时间减少患者的住院费用。

Objectives This study aims to analyze the factors affecting the hospitalization and hospitalization expenses of patients with oral cancer,so as to provide a reference for rationally controlling medical expenses and reducing the economic burden of patients.Methods Information from the front page of medical records with the first diagnosis of oral cancer at a certain hospital from January 1,2017 to December 31,2021 was collected.Statistical methods such as descriptive analysis,new gray relational analysis,structural change,analysis,and multiple linear regression were used to analyze the basic situation and hospitalization expenses of patients with oral cancer.Results The average hospitalization cost of 419 patients was 15008.20 yuan.Diagnosis accounted for the largest proportion(27.52%),followed by western medicine(25.65%)and treatment(21.69%).The average annual structural change was 13.90%.The main categories affecting structural changes were diagnostics,consumables,and western medicines,with a cumulative contribution rate of 81.57%.The diagnostic class had a positive change.Consumables and western medicines showed negative changes.The top three correlation coefficients of each category of hospitalization expenses for patients with oral cancer from large to small were diagnosis,western medicine,and treatment.The factors affecting the hospitalization expenses from large to small were the scope of surgery,length of hospital stay,year of discharge,and the number of discharge diagnoses.Conclusions The structure of hospitalization expenses for patients with oral cancer is gradually becoming more reasonable,but diagnostic expenses are still the focus of adjustment.In addition,the hospitalization expenses of patients can be reduced by reasonable control of hospitalization time.
