某三级中医医院2012年-2021年住院死亡病例分析

Analysis of Inpatient Deaths in a Tertiary Hospital of Traditional Chinese Medicine from 2012 to 2021

目的对某三级中医医院的死亡病例进行统计分析,为完善资源配置提高诊疗能力、提升服务水平、降低病死率提供参考依据。方法编写SQL语句收集某中医医院电子病历系统中2012年1月1日至2021年12月31日的1689例死亡患者的住院病案首页数据,使用SPSS23.0软件进行描述性分析及统计分析,统计前十位死因,采用Cox-Stuart趋势检验对十年间患者病死率进行检验,采用卡方检验方法对不同性别、不同年龄组病死率进行组间比较。结果10年病死率为0.52%,男性患者整体病死率(0.68%)高于女性(0.39%),老年男性(50岁~89岁)的病死率远高于女性,不同性别、不同年龄段患者的死亡人数分布差异有统计学意义(P<0.01)。死因前三位的分别为肿瘤、呼吸系统疾病、循环系统疾病,占比分别为56.54%、16.34%、12.49%。结论最近10年,住院患者病死率呈下降趋势。肿瘤死因前五位的依次是支气管和肺恶性肿瘤、肝和肝内胆管恶性肿瘤、结肠恶性肿瘤、胃恶性肿瘤、直肠恶性肿瘤。呼吸系统疾病首位死因是慢性阻塞性肺疾病。医疗机构应着力提升肿瘤疾病的诊疗能力与服务水平,降低病死率。

Objectives To analyze the death cases in a three-level traditional Chinese medicine hospital,and provide reference basis for improving resource allocation,improving diagnosis and treatment capacity,improving service level and reducing mortality.Methods To write SQL statements to collect data of 1689 dead patients from EMR system in a traditional Chinese medicine hospital from 2012 to 2021,use SPSS23.0 software for descriptive analysis and statistical analysis,calculate the top ten causes of death,use Cox Stuart trend test to test the mortality of patients in ten years,use Chi-square test to compare the mortality of different gender and age groups.Results The 10-year mortality was 0.52%,the overall mortality of male patients was higher than that of female patients,the mortality of elderly men was much higher than that of women,there were significant differences in the distribution of deaths among patients of different genders and ages,the top three causes of death were tumors,respiratory diseases,circulatory diseases,accounted for 56.54%,16.34%and 12.49%respectively.Conclusions In the last 10 years,the mortality of hospitalized patients showed a downward trend,the top five causes of death were lung malignancy,liver malignancy,colon malignancy,stomach malignancy and rectal malignancy,the leading cause of death from respiratory diseases was chronic obstructive pulmonary disease,medical institutions should strive to improve the diagnosis and treatment capacity and service level of tumor diseases,reduce the mortality.
