时间观疗法对颌面创伤患者病耻感和心理弹性的影响

Effect of time perspective therapy on stigma and mental resilience of patient with maxillofacial trauma

目的探讨时间观疗法在颌面创伤患者中的应用效果。方法选取2021年1月至2022年1月收治的86例颌面创伤患者,将其随机分为试验组和对照组,每组43例。在常规护理基础上,对照组给予认知行为干预,试验组实施时间观疗法。干预前和干预6周后,采用社会影响量表(SIS)和心理弹性量表(CD-RISC)进行评价。结果干预前,2组患者SIS评分和CD-RISC评分差异无统计学意义(P>0.05)。干预后,试验组社会排斥、羞耻感、社会隔离等维度评分和SIS总分分别为(18.00±1.85)分、(8.95±1.32)分、(15.90±1.45)分和(50.00±3.13)分,低于对照组的(19.64±2.16)分、(10.62±1.86)分、(16.98±1.96)分和(54.83±3.85)分;试验组坚韧、自强、乐观等维度评分和CD-RISC总分分别为(27.80±4.32)分、(20.63±5.47)分、(11.10±2.99)分和(59.53±7.81)分,高于对照组的(24.74±4.48)分、(18.21±4.64)分、(9.62±2.71)分和(52.57±7.52)分,差异均有统计学意义(P<0.05)。结论时间观疗法可有效降低颌面创伤患者病耻感并提高其心理弹性水平。

Objective To investigate the outcomes of time perspective therapy on stigma and mental resilience of patients with maxillofacial trauma.Methods A total of 86 maxillofacial trauma patients were randomly allocated to experimental group(n=43)and control group(n=43).Based on the conventional nursing care,cognitive behavioral intervention and time perspective therapy were given to patients in both groups,respectively.Before and after 6 months of the intervention,the Social Impact Scale(SIS)and Connor and Davidson Resilience Scale(CD RISC)were used for assessments.Results Before the intervention,there was no significant difference in the SIS and CD-RISC scores between groups(P>0.05).After the intervention,scores in the dimensions of social exclusion(18.00±1.85 vs 19.64±2.16),internal shame(8.95±1.32 vs 10.62±1.86),and social isolation(15.90±1.45 vs 16.98±1.96),and total SIS scores(50.00±3.13 vs 54.83±3.85)in the experimental group were significantly lower than those in the control group(P<0.05).Scores in the dimensions of tenacity(27.80±4.32 vs 24.74±4.48),strength(20.63±5.47 vs 18.21±4.64),and optimism(11.10±2.99 vs 9.62±2.71),and total CD-RISC scores(59.53±7.81 vs 52.57±7.52)in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion Time perspective therapy can effectively alleviate stigma and promote mental resilience of patients with maxillofacial trauma.
