摘要
目的分析AI术前三维规划对全髋关节置换术(THA)预测假体型号及髋臼假体位置精准性的影响。方法回顾性收集术前使用AIHIP软件辅助完成初次THA的患者共148例,列为AI规划组;术前采用传统X线二维规划辅助完成THA的患者共87例,列为传统规划组。比较2组术前规划预测假体型号准确率,髋臼假体的外展角、前倾角及位于Lewinnek安全区、Callanan安全区的比例。结果AI规划组预测假体型号的准确率明显高于传统规划组(P<0.05);AI规划组与传统规划组术后髋臼杯假体的外展角、前倾角的差异均无统计学意义(P>0.05);AI规划组术后髋臼假体位于Lewinnek安全区及Callanan安全区的比例均高于传统规划组(P<0.05)。结论AI术前三维规划在预测假体型号方面明显优于传统X线胶片模板二维规划,提高了髋臼假体植入的准确性及髋臼假体位于安全区内的比例。
Objective To analyze the effect of Artificial Intelligence-assisted three-dimensional preoperative planning technology on the accuracy of predicting prosthesis type and acetabular prosthesis position for total hip arthroplasty(THA).Methods 148 patients who completed initial THA with the assistance of AIHIP software preoperatively were retrospectively collected and classified as the observation group.87 patients who completed THA with the aid of conventional X-ray 2D planning preoperatively were included as the control group.The accuracy of preoperative planning of the predicted prosthesis type,the abduction angle and anteversion angle of the acetabular prosthesis and the proportion located in the Lewinnek safety zone and Callanan safety zone were compared between two groups.Perioperative information was analyzed,such as the accuracy of the planned size with the actual prosthesis size,the postoperative inclination and the anteversion of the acetabular cup,the rate of acetabular cup within the Lewinnek safe zone and Callanan safe zone.Results The accuracy of preoperative planning of acetabular prosthesis and femoral prothesis were both significantly higher in observed group(P<0.05).There was no statistically significantly difference between two groups in inclination angle and anteversion angle(P>0.05).The rate of acetabular cup within the Lewinnek safe zone and Callanan safe zone were both higher in observed group(P<0.05).Conclusion AI-assisted 3D preoperative planning is obviously better than traditional 2D X-ray template planning in predicting prosthesis size.Meanwhile it reveals advantage in acetabular cup positioning.
