基于真实世界研究探讨针灸治疗脑卒中后睡眠障碍临床疗效

Analysis of Clinical Efficacy of Acupuncture and Moxibustion in Treatment of Patients with Sleep Disorders After Stroke Based on Real-World Data

目的:基于真实世界数据分析针灸治疗脑卒中后睡眠障碍的临床疗效,为针灸治疗本病提供可靠的循证医学证据。方法:根据是否采用针灸治疗,将261例脑卒中后睡眠障碍患者分为观察组145例和对照组116例,根据倾向性评分值及1∶1匹配原则进行配对,最终成功匹配96对病例。比较匹配后两组患者的中医证候积分、阿森斯失眠量表(AIS)评分、匹兹堡睡眠质量指数(PSQI)评分及临床疗效。结果:治疗后,两组中医证候积分、AIS评分、PSQI评分均较治疗前降低,且观察组低于对照组,差异具有统计学意义(P<0.05);总有效率分别为观察组87.50%、对照组76.04%,观察组疗效优于对照组(P<0.05)。结论:基于真实世界的数据表明针灸治疗脑卒中后睡眠障碍有效,可以改善患者睡眠质量,提高生活质量。

Objective:To analyze the clinical efficacy of acupuncture and moxibustion in the treatment of patients with sleep disorders after stroke based on real-world data,for reliable evidence-based medical data.Methods:According to whether the patients were treated with acupuncture and moxibustion or not,261 patients with sleep disorders after stroke were divided into two groups,145 cases in observation group and 116 cases in control group.They were matched according to the propensity score and the 1∶1 matching principle.Finally,96 pairs of cases were successfully matched,96 cases in each group were selected.The clinical efficacy,traditional Chinese medicine(TCM)syndrome score,Athens Insomnia Scale(AIS)score and Pittsburgh sleep quality index(PSQI)score of the two groups after matched were compared.Results:After treatment,TCM syndrome score,AIS score and PSQI score in both groups were lower than before treatment,and observation group were lower than control group,the differences were statistically significant(P<0.05).The total effective rates were 87.50%in the observation group and 76.04%in the control group.The efficacy of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Evidence based on the real world shows that acupuncture and moxibustion is effective in the treatment of patients with sleep disorders after stroke,improving sleep quality and improving quality of life in patients.
