四位一体疗法干预特发性矮小症儿童生长的基础研究

Basic research on the intervention of four-in-one therapy on the growth of children with idiopathic short stature

目的 探讨四位一体疗法治疗特发性矮小(ISS)儿童的临床疗效。方法 选取ISS儿童60例,随机分为四位一体组、重组人生长激素(rhGH)组、对照组,各20例。治疗前、治疗3个月后、治疗6个月后检查患儿身高、骨龄(BA),比较患儿年生长速率(HV)、胰岛素样生长因子1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP3)变化情况。结果四位一体组、rhGH组治疗6个月后HV较治疗前明显升高(P <0.05),四位一体组与对照组HV差值2.71 cm/年,rhGH组与对照组HV差值2.96 cm/年。四位一体组、rhGH组在治疗6个月后IGF-1与IGFBP3较治疗前明显升高(P <0.05)。四位一体组、rhGH组与对照组BA进展差异无统计学意义(P> 0.05)。3组6个月的治疗中均未见严重不良反应。结论 四位一体疗法治疗ISS疗效确切,值得临床推广。

Objective To observe the therapeutic effect of the four-in-one traditional Chinese medicine method on children with idiopathic short stature(ISS).Methods A total of 60 children with ISS were selected and divided into the four-in-one therapy group,the recombinant human growth hormone(rhGH)group,and the control group,with 20 cases each.Before treatment,at 3 and 6 months after treatment,the height and bone age(BA)of the children were checked,and their height velocity(HV),insulin-like growth factor 1(IGF-1)and the insulin-like growth factor binding protein-3(IGFBP3)were compared.Results After 6 months of treatment,the HV of the four-in-one therapy group and the rhGH group was significantly higher than that before treatment(P<0.05).The difference in HV between the four-in-one therapy group and the control groups was 2.71 cm/year,and the difference in HV between the rhGH group and the control group was 2.96 cm/year.After 6 months of treatment,the levels of IGF-1 and IGFBP3 were significantly higher in the four-in-one therapy group and the rhGH group compared with before treatment(P<0.05).There was no significant difference in BA progression between the four-in-one therapy group,the rhGH group and the control group(P>0.05).There were no serious adverse reactions in three groups of children with ISS during the 6-month treatment.Conclusion The four-in-one treatment method for ISS is effective and worthy of clinical promotion.
