芒针透刺躯干肌联合穴位敷贴治疗中风后平衡障碍

Treatment of balance disorder after stroke with elongated needle piercing through the trunk muscle combined with acupoint application

目的 探究芒针透刺躯干肌联合穴位敷贴对中风后平衡障碍的治疗效果。方法 选取我院2020年1月-2021年12月收治的中风后平衡障碍患者60例,随机分为观察组和对照组,各30例,对照组采用西医常规治疗联合穴位敷贴,观察组采用西医常规治疗和芒针透刺躯干肌联合穴位敷贴,持续4 w,评价治疗前、治疗后2 w和治疗后4 w 2组患者Berg评分(BBS)、Fugl-Meyer评分及Barthel指数,平衡测试仪检测质心轨迹面积和长度差异,比较2种疗法疗效及安全性。结果 治疗后,2组患者BBS、Fugl-Meyer评分和Barthel指数均随治疗时间延长而提高(P <0.05),质心轨迹面积和长度均随治疗时间延长而降低(P <0.05),组内不同时间点比较差异有统计学意义(P <0.05),治疗后2 w和治疗后4 w,观察组均优于对照组(P <0.05);观察组患者临床有效率为86.66%,高于对照组的63.33%(P <0.05),治疗期间2组患者不良事件发生率无统计学意义(P> 0.05)。结论 芒针透刺躯干肌联合穴位敷贴可改善患者中风后平衡障碍,增强患者运动能力,效果优于单独使用穴位敷贴,且具有安全性高的优点。

Objective To explore the therapeutic effects of elongated needle piercing through the trunk muscle combined with acupoint application on post-stroke balance disorder.Methods 60 patients with post-stroke balance disorder who were treated in the Second Affiliated Hospital of Anhui University of Traditional Chinese Medicine were selected from January 2020 to December 2021,and randomly divided into the observation group and the control group with 30 cases in each group.The control group received conventional western medicine treatment combined with acupoint application,and the observation group was treated with conventional western medicine treatment and elongated needle piercing through the trunk muscle combined with acupoint application,and both group were treated for 4 weeks.Before treatment,at 2 weeks after treatment and 4 weeks after treatment,Berg balance score(BBS),Fugl-Meyer score and Barthel index,and the area and length differences of mass center orbit detected by balance tester were assessed respectively.Eficacy and safety of the two treatment methods were compared.Results After treatment,the BBS score,Fugl-Meyer score and Barthel index of 2 groups were increased with the prolongation of treatment duration(P<0.05),and the area and length of mass center orbit were reduced or shortened with the prolongation of treatment duration(P<0.05).There were statistically differences at different time points among the groups(P<0.05).And the indicators above in the observation group were better than those in the control group at 2 weeks and 4 weeks after treatment(P<0.05).The clinical effective rate in the observation group(86.66%)was significantly higher than that in the control group(63.33%)(P<0.05).There was no statistically differences in the incidence of adverse events between 2 groups during treatment(P>0.05).Conclusion Elongated needle piercing through the trunk muscle combined with acupoint application can improve the balance disorder after stroke and enhance the exercise ability.The effect is better than using acupoint application alone.And the treatment is safe.
