督脉灸联合针刀及穴位贴敷治疗寒湿型腰椎间盘突出症的临床疗效观察

Clinical efficacy of governor vessel moxibustion combined with akupotomye and acupoint application in the treatment of lumbar disc herniation of cold-damp type:an observation study

目的探讨督脉灸联合针刀及穴位贴敷治疗寒湿型腰椎间盘突出症(LDH)的临床疗效。方法将90例寒湿型LDH患者随机分为对照组和观察组,各45例。对照组予以常规西医治疗,观察组在对照组基础上增加督脉灸联合针刀及穴位贴敷治疗,两组均连续治疗2周。比较两组的临床疗效、治疗前后的疼痛程度(VAS评分)、腰椎功能[Oswestry功能障碍指数(ODI)]、疼痛介质水平(前列腺素E_(2)和5-羟色胺)及治疗期间的不良反应发生情况。结果观察组的临床总有效率高于对照组,治疗后的ODI评分、VAS评分、前列腺素E_(2)水平、5-羟色胺水平均低于对照组(均P<0.05)。两组治疗期间均未发生明显不良反应。结论应用督脉灸联合针刀及穴位贴敷治疗寒湿型LDH的疗效确切,可提高临床疗效,减轻患者疼痛,改善腰椎功能,降低前列腺素E_(2)、5-羟色胺水平,值得临床推广应用。

Objective To investigate the clinical efficacy of governor vessel moxibustion combined with akupotomye and acupoint application in the treatment of lumbar disc herniation(LDH)of cold-damp type.Methods A total of 90 patients with LDH of cold-damp type were randomly divided into a control group and an observation group,with 45 cases in each group.The control group was treated with conventional western medicine,and the observation group was additionally treated with governor vessel moxibustion combined with akupotomye and acupoint application on the basis of the control group′s therapy.Both groups received continuous treatment for 2 weeks.The clinical efficacy,pain severity(VAS score),lumbar vertebra function[Oswestry disability index(ODI)],pain mediator levels(prostaglandin E_(2)and 5-hydroxytryptamine)before and after treatment,and occurrence of adverse reactions during treatment were compared between the two groups.Results The clinical total effective rate of the observation group was higher than that of the control group,and the ODI score,VAS score,prostaglandin E_(2)level and 5-hydroxytryptamine level after treatment were lower than those of the control group(all P<0.05).No obvious adverse reactions occurred in the two groups during treatment.Conclusion The application of governor vessel moxibustion combined with akupotomye and acupoint application is effective in treating LDH of cold-damp type,which can improve clinical efficacy,relieve pain,improve lumbar vertebra function and reduce the levels of prostaglandin E_(2)and 5-hydroxytryptamine for the patients,and is worthy of clinical promotion and application.
