Serotype distribution,drug resistance and molecular typing of Salmonella in patients of diarrhea in Luoyang,Henan province from 2017 to 2021

摘要 目的分析河南省洛阳市腹泻患者沙门氏菌的血清型分布、耐药状况及分子分型,为食源性疾病防控工作提供依据。方法收集2017-2021年洛阳市2家哨点医院944份腹泻患者粪便样本,进行沙门氏菌的分离鉴定和血清学分型;采用微量肉汤稀释法进行抗生素敏感性试验,通过脉冲场凝胶电泳(PFGE)进行分子分型聚类分析。结果944份样本共检出70株沙门氏菌,检出率7.4%,其中3岁以下患者最常见(37.1%)。共鉴定出17个血清型,肠炎沙门氏菌占据优势,占47.1%。对氨苄西林和萘啶酸的耐药率最高,分别为85.7%和78.6%;有12株菌对10种及以上抗生素全耐药,仅亚胺培南对所有菌株均敏感;多重耐药菌共59株,占84.3%。PFGE聚类分析相似度为49.2%,分为43种型别,部分不同血清型沙门氏菌PFGE型别相似。结论洛阳市腹泻患者检出的沙门氏菌血清型和基因型呈现多样化,存在多人感染同一基因型的情况,菌株多重耐药情况严重。 Objective To analyze the serotype distribution of Salmonella in patients of diarrhea in Luoyang,Henan,and provide the evidence for the prevention and control of foodborne diseases through the study of drug resistance and molecular typing.Methods From 2017 to 2021,944 stool samples of diarrhea patients from 2 sentinel hospitals in Luoyang were collected for isolation,identification and serological typing of Salmonella.The molecular typing cluster analysis was carried out by pulsed field gel electrophoresis(PFCE),and the antibiotic sensitivity test was carried out by micro broth dilution method.Results A total of 70 strains of Salmonella were detected,with a detection rate of 7.4%.The most common was patients under 3 years old(37.1%).A total of 17 serotypes were identified,and Salmonella Enteritidis was dominant,accounting for 47.1%.The highest rates of resistance to ampicillin and naphthyric acid were 85.7%and 78.6%,respectively;12 strains were all resistant to 10 or more antibiotics,and only imipenem was sensitive to all strains.There were 59 strains of multidrug resistant bacteria,accounting for 84.3%.The similarity of PFCE cluster analysis was 49.2%,which was divided into 43 types,and PFGE types for some different serotypes of Salmonella were similar.Conclusions In Luoyang,the serotypes and genotypes of Salmonella identified from the patients of diarrhea are diverse.There are many people infected with the same genotype,and the situation of multiple drug resistance of the strains is serious.
