负压封闭引流在骨科创伤患者中的应用效果分析

Analysis of Curative Effect of Vacuum Sealing Drainage in Treating Patients with Orthopedic Trauma

目的探讨负压封闭引流在骨科创伤患者中的应用效果。方法选取2020年6月至2021年6月洛阳市偃师人民医院收治的96例骨科创伤患者作为研究对象,按照随机数表法将其随机分为观察组(48例)和对照组(48例),观察组患者局部创面采用负压封闭引流治疗,对照组患者局部创面采用凡士林油纱治疗,对比观察两组患者血清炎症因子水平、治疗14 d时创面愈合质量、创面愈合时间及并发症发生情况。结果治疗7 d时,观察组患者血清C反应蛋白(CRP)、白细胞计数(WBC)、肿瘤坏死因子⁃α(TNF⁃α)水平均明显低于对照组(t=11.141、4.143、5.344,P均<0.001);治疗14 d时,观察组患者创面愈合质量为优24例、良19例、中2例、差3例,明显优于对照组患者的创面愈合质量为优17例、良14例、中8例、差9例(Z=-2.279,P=0.023);观察组患者创面愈合时间为(19.42±3.56)d,明显短于对照组患者的创面愈合时间(23.85±4.50)d(t=5.349,P<0.001);治疗期间,观察组患者并发症发生率为10.42%,与对照组患者的并发症发生率20.83%无明显差异(χ^(2)=1.975,P=0.160)。结论负压封闭引流可明显降低骨科创伤患者炎症反应程度,提高创面愈合质量,缩短创面愈合时间,疗效显著。

Objective To study the curative effect of vacuum sealing drainage in treating patients with orthopedic trauma.Methods 96 patients with orthopedic trauma,admitted into Yanshi People’s Hospital,Luoyang,from June 2020 to June 2021 were enrolled and divided into study group(n=48)and control group(n=48)using the random number table.The local wounds of patients in the study group were treated with vacuum sealing drainage,while the local wounds of patients were treated with Vaseline gauze in the control group.The levels of serum inflammatory factors,wound healing quality on day 14 of treatment,wound healing time and occurrence of complication were compared between the two groups.Results On day 7 of treatment,the levels of serum C⁃reactive protein(CRP),white blood cell count(WBC)and tumor necrosis factor⁃α(TNF⁃α)of patients were obviously lower in the study group compared with the control group(t=11.141,4.143 and 5.344,all P<0.001).On day 14 of treatment,the wound healing quality was evaluated as excellent in 24 cases,good in 19 cases,fair in 2 cases and poor in 3 cases in the study group,which was significantly better than the control group(Z=-2.279,P=0.023)⁃excellent in 17 cases,good in 14 cases,fair in 8 cases and poor in 9 cases.The wound healing time of patients was(19.42±3.56)days in the study group,being obviously shorter than the corre⁃sponding(23.85±4.50)days in the control group(t=5.349,P<0.001).During the course of treatment,the incidence of complications was 10.42%in the study group,which showed no statistically significant difference compared with the corresponding 20.83%in the control group(χ^(2)=1.975,P=0.160).Conclusion Vacuum sealing drainage can significantly alleviate the inflammatory responses of patients with orthopedic trauma,enhance wound healing quality,and shorten wound healing time,and the curative effect is definite.
