血清25-羟基维生素D水平与帕金森病患者认知功能减退的相关性 被引量:5

摘要 目的 探讨血清25-羟基维生素D[25(OH)D]水平与帕金森病(PD)患者认知功能减退(CD)的相关性。方法 将西南交通大学附属医院神经内科1 16例PD患者根据MoCA评分分为认知功能正常组(PD-NC组)和认知功能减退组(PD-CD组)收集两组基础资料、病情资料、实验室指标及血清25(OH)D水平,分析PD患者发生CD的危险因素,研究PD患者血清25(OH)D水平与MoCA评分的相关性结果 PD-CD组患者各项MoCA评分均低于PD-NC组(P<0.05) PD-CD组病程较PD-NC组更长,Hoehn-Yahr(H-Y)分期也更高(P<0.05);PD-CD组患者血清同型半胱氨酸(Hcy)水平高于PD-NC组,血清25(OH)D水平则低于PD-NC组(P<0.05)多因素Logistic回归显示,病程、H-Y分级、血清25(OH)D及Hcv水平是PD患者发生CD的独立危险因素(P<0.05)血清25(OH)D水平与MoCA总评分呈显著正相关关系(P<0.05)。结论 血清25(OH)D水平与PD患者认知功能状况密切相关,可为PD患者CD的诊断和治疗提供参考。 Objective To invesligate the xorrelation between serum 25-hydroxyvilamin D [25(OH)D]level and cognitive decline(CD) in patients with Parkinson's disease(PD).Methods 116 PD patients in affiliated hospital of southwest jiaotong universily were divided into normal cognitive function group(PD-NC group) and cognitive decline group(PD-CD group) according to MoCA score.Basic data,disease information,laboratory indicators and the serum 25(OH)D level of the two groups were collected.The risk factors of CD in PD patients were analyzed.The correlation between serum 25(OH)D level and MoCA score in PD patients was studied.Results All MoCA scores in PD-CD group were lower than those in PD-NC group(P<0.05).The course of disease in PD-CD group was longer than that in PD-NC group,and Hoehn-Yahr(H-Y) stage was higher(P<0.05).The level of serum homocysteine(Hcy) in PD-CD group was higher than that in PD-NC group,while the level of serum 25(0H)D was lower than that in PD-NC group(P<0.05).Multivariale Logistic regression showed that the course of disease,H-Y grade,serum 25(OH)D and Hcy level were independent risk factors for CD in PD patients(P<0.05).There was a significant positive correlation between serum 25(OH)D level and total MoCA score(P<0.05).Conclusions The level of serum 25(OH)D is closely related to the cognitive function of PD patients,which can provide reference for the diagnosis and treatment of CD in patients.
