以面部红肿为表现的上腔静脉综合征

Superior vena cava syndrome manifested as facial redness:a case report

报告1例上腔静脉综合征。患者男,66岁。面部及颈部红肿半个月余。皮肤科检查:面部弥漫性红斑、肿胀,双眼睑水肿;颈部及胸部皮肤潮红,可见散在分布的蓝紫色蚓状静脉曲张,微隆起于皮面。影像学检查见纵隔占位,组织病理检查示核深染的异形细胞巢团,挤压明显。免疫组化:CD56(+)及增殖核抗原(Ki-67)(90%+)。诊断:由小细胞肺癌所致的上腔静脉综合征。

A case of superior vera cava syndrome is reported.A 66-year-old male presented with redness and swelling of his face and neck for more than half a month.Physical examination showed diffuse erythema and swelling of the face,edema of both eyelids.The skin on the neck and chest were flushed,with scattered blue-purple vermiform varicose veins,slightly bulging on the skin.Radiology examination showed mediastinal space-occupying lesions.Histopathological examination revealed nests of heteromorphic cells with the nuclei that were deeply stained.The extrusion was obvious.Immunohistochemistry showed that the tumor cells were positive for CD56 and Ki-67(90%).The patient was diagnosed as superior vena cava syndrome caused by small cell carcinoma of the lung.
