家庭药师应用《T2DM患者居家药物治疗管理服务中MTRPs评估指引》的效果研究

Study on the Effect of"Guidelines for Evaluation of MTRPs in Home Medication Management Service for T2DM Patients"Applied by Family Pharmacists

目的探讨家庭药师应用《T2DM患者居家药物治疗管理服务中MTRPs评估指引》(以下简称《评估指引》)的效果。方法专家团队建立《评估指引》,筛选具有MTRPs代表性的T2DM患者20例,采集完整信息并填写患者“用药相关信息记录表”,应用《评估指引》对患者进行评估;佛山市南海区中各级医疗机构随机选择家庭药师代表12个,在应用《评估指引》干预前后,各自单独对20例T2DM患者进行适应证、有效性、安全性、依从性、用药相关检查和生活方式等6方面的MTRPs评估,记录各类型问题的评估数量、时长和准确率;比较:(1)干预前后,家庭药师对T2DM患者MTRPs评估数量、时长和准确率的差异;(2)干预后,家庭药师和专家团队对T2DM患者MTRPs评估数量和时长的差异。结果应用《评估指引》后,家庭药师对T2DM患者MTRPs评估数量和准确率均较干预前有明显的提升,家庭药师发现问题的数量明显少于专家团队、评估时长也高于专家团队,但干预前后家庭药师评估问题的总时长没有差异。结论《评估指引》在一定程度上有利于快速提升基层家庭药师评估T2DM患者MTRPs的能力,使家庭药师提供的服务高质量、高效率。

OBJECTIVE To probe the effects of family pharmacists in applying the Guidelines for the Evaluation of MTRPs in Home Medication Management Services for Patients with T2DM(hereinafter referred to as the Evaluation Guidelines).METHODS The expert team established the Evaluation Guide and screened 20 patients with T2DM with representative of MTRPs,collected complete information and filled in“the Medication⁃related Information Record Form”of the patients,and applied the Evaluation Guidelines to evaluate the patients.Twelve representatives of family pharmacists were randomly selected by medical institutions at all levels in Nanhai district of Foshan City,before and after the application of the Evaluation Guidelines intervention.Twenty patients with T2DM were separately evaluated for MTRPs in six aspects,including indications,effectiveness,safety,compliance,medication related examination and lifestyle,and the problems’number,duration and accuracy of evaluation for each type of problems were recorded.Comparison:①Differences in the problems’number,duration and accuracy of MTRPs evaluated by family pharmacists in T2DM patients before and after the application of Evaluation Guidelines.②Differences in the problems’number and time length of MTRPs assessed by family pharmacists and expert teams in patients with T2DM after the application of Evaluation Guidelines.RESULTS After the application of Evaluation Guidelines,the number and accuracy of MTRPs evaluation of T2DM patients by family pharmacists were significantly improved compared with that before intervention,the number of problems found by family pharmacists were significantly less than that of the expert team,and the evaluation time was also higher than that of the expert team.But there was no difference in the total time family pharmacists spent assessing problems before and after the intervention.CONCLUSION To a certain extent,the Evaluation Guidelines are conducive to rapidly improving the ability of grassroots family pharmacists to evaluate MTRPs in T2DM patients,so as to make the services provided by family pharmacists high quality and high efficiency.
