基于红外热成像技术评价浮针治疗肩周炎的疗效观察

Evaluation of Therapeutic Effect of Floating Needle on Scapulohumeral Periarthritis Based on Infrared Thermography

目的:通过红外热成像技术,观察浮针治疗肩周炎的临床疗效。方法:将60例肩周炎患者随机分为治疗组、对照组1、对照组2各20例。治疗组采取浮针治疗,对照组1采取电针治疗,对照组2采取口服美洛昔康治疗。比较3组患者治疗前和治疗6 d后疼痛分级指数(PRI评分)、视觉模拟评分(VAS评分)、现有疼痛强度(PPI评分)及肩关节活动范围(ROM),统计3组患者治疗前和治疗6 d后热成像的肩髃、肩髎、臂臑穴位温度,并评价临床疗效。结果:治疗组总有效率高于对照组1、对照组2(P<0.05);对照组1总有效率与对照组2比较,差异无统计学意义(P>0.05)。3组患者治疗6 d后PRI评分、VAS评分和PPI评分均低于治疗前(P<0.05),肩关节ROM均高于治疗前(P<0.05),肩髃、肩髎、臂臑穴位温度均低于治疗前(P<0.05);治疗组患者治疗6 d后PRI评分、VAS评分和PPI评分均低于对照组1、对照组2(P<0.05),肩关节ROM高于对照组1、对照组2(P<0.05),肩髃、肩髎、臂臑穴位温度均低于对照组1、对照组2(P<0.05)。结论:浮针治疗肩周炎镇痛效果显著,能扩大肩关节活动度;红外热成像可作为研究浮针治疗肩周炎的客观敏感指标。

Objective:To observe the clinical effect of floating needle on scapulohumeral periarthritis through infrared thermal imaging technology.Methods:Totally 60 patients with scapulohumeral periarthritis were randomly divided into treatment group,control group 1 and control group 2,with 20 cases in each group.Patients were treated by floating acupuncture in treatment group,electroacupuncture in control group 1 and oral Meloxicamthe in control group 2.The pain rating index(PRI),visual analogue score(VAS),current pain intensity(PPI)and shoulder joint range of motion(ROM)were compared before treatment and after 6 days of treatment in the three groups.The temperature of Jianyu(LI 15),Jianliao(TE 14)and Bimao(LI 14)was measured by infrared thermal imaging technology before treatment and after 6 days of treatment,and the treatment efficiency was evaluated.Results:The treatment group showed higher total effective rate than control group 1 and control group 2(P<0.05).There was no significant difference in total effective rate between control group 1 and control group 2(P>0.05).After 6 days of treatment,the three groups showed lower PRI,VAS and PPI scores than before treatment(P<0.05),larger ROM than that before treatment(P<0.05),while lower temperature of Jianyu(LI 15),Jianliao(TE 14)and Bimao(LI14)than that before treatment(P<0.05).After 6 days of treatment,the treatment group showed lower PRI,VAS and PPI scores than control group 1 and control group 2(P<0.05),larger ROM than control group 1 and control group 2(P<0.05),while lower temperature of Jianyu(LI 15),Jianliao(TE 14)and Bimao(LI14)than control group 1 and control group 2(P<0.05).Conclusion:The analgesic effect of floating needle on periarthritis of shoulder is significant,and the range of motion of shoulder joint is effectively expanded.Infrared thermography can be used as an objective and sensitive index for studying the treatment of periarthritis of shoulder with floating needle.
