基于隐结构结合系统聚类的儿童社区获得性肺炎中医常见证候特征研究

Study on the Common Manifestation of Syndrome Characteristic of Traditional Chinese Medicine of Children with Community-acquired Pneumonia Based on Latent Structure Binding System Clustering Analysis

目的:基于隐结构模型结合系统聚类的方法,探讨儿童社区获得性肺炎的中医证候特征,为临床辨证分型提供参考。方法:检索中文数据库收录的儿童社区获得性肺炎证候相关文献,经筛选后,采用Lantern 5.0软件隐结构法和SPSS 25.0软件因子分析及聚类分析,结合证名及症的频数、频率,得出儿童社区获得性肺炎常见中医证型及相应症。结果:纳入文献373篇,包括654条证名记录及108个症。隐结构分析得出风寒闭肺证、风热闭肺证、痰热闭肺证、热邪闭肺证、毒热闭肺证、阴虚肺热证、肺脾气虚证、气阴两虚证8个常见证型。系统聚类分析推断出6个潜在证型,分别为风寒闭肺证、痰热闭肺证、热邪闭肺证、毒热闭肺证、阴虚肺热证、肺脾气虚证。结合构成比>5%的证名及对应构成比>30%的症,总结儿童社区获得性肺炎的8个常见证型及其主要症。结论:儿童社区获得性肺炎共8个常见证型,分别为风寒闭肺证、风热闭肺证、痰热闭肺证、热邪闭肺证、毒热闭肺证、阴虚肺热证、肺脾气虚证、气阴两虚证。

Objective:Based on the latent structure model combined with systematic clustering method,to explore the common manifestation of syndrome characteristic of traditional Chinese medicine(TCM)of community-acquired pneumonia in children,and provide reference for clinical syndrome differentiation and classification.Methods:The literature related to the manifestation of syndrome of community acquired pneumonia in children was retrieved from Chinese database.After screening,Lantern 5.0 software latent structure method and SPSS 25.0 software factor analysis and cluster analysis were used,combined with the frequency and frequency rate of name of syndrome,symptoms and signs,to identify common traditional Chinese medicine types of syndrome and corresponding symptoms and signs of community acquired pneumonia in children.Result:A total of 373 literatures were included,including 654 name of syndrome records and 108 symptoms and signs.The latent structure analysis revealed 8 common types of syndrome:wind and cold closed lung syndrome,wind and heat closed lung syndrome,phlegm-heat closed lung syndrome,heat evil closed lung syndrome,toxic heat closed lung syndrome,Yin deficiency lung heat syndrome,lung-spleen Qi deficiency syndrome and Qi Yin deficiency syndrome.6 potential types of syndrome types were deduced by systematic cluster analysis,which were wind and cold closed lung syndrome,phlegm-heat closed lung syndrome,heat evil closed lung syndrome,toxic heat closed lung syndrome,Yin deficiency lung heat syndrome,and lung-spleen Qi deficiency syndrome.Combined with the names of syndrome with the constituent ratio>5%and the corresponding symptoms and signs with the constituent ratio>30%,8 common types of syndrome and their main symptoms and signs of community-acquired pneumonia in children were summarized.Conclusion:There are 8 common types of syndrome of community-acquired pneumonia in children,including wind and cold closed lung syndrome,wind and heat closed lung syndrome,phlegm-heat closed lung syndrome,heat evil closed lung syndrome,toxic heat closed lung syndrome,Yin deficiency lung heat syndrome,lung-spleen Qi deficiency syndrome and Qi Yin deficiency syndrome.
