血清miR135a-5p在肺癌患者辅助诊断中的应用价值

摘要 目的探讨肺癌患者血清miR135a-5p表达水平及其在肺癌诊断中的临床价值。方法采集115例肺癌患者(肺癌组)、100例肺部良性疾病患者(良性组)和100例健康体检者(对照组)血清样本量,检测其血清miR135a-5p、癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白片段19(CYFRA21-1)水平,应用受试者工作曲线(ROC)评价miR135a-5p、CEA、NSE、CYFRA21-1对肺癌及其不同病理类型的辅助诊断效能。结果肺癌组血清CEA、NSE、CYFRA21-1、miR135a-5p水平中位数分别为4.68 ng/ml、15.27 ng/ml、5.03 ng/ml、2.83,高于良性组和对照组(3.89 ng/ml、14.50 ng/ml、4.27 ng/ml、1.21和2.72 ng/ml、11.75 ng/ml、2.93 ng/ml、1.06),差异有统计学意义(P<0.05)。血清miR135a-5p含量与病理类型、淋巴结转移、临床分期有关(P<0.05),与年龄、肿瘤最大径无关(P>0.05)。血清miR135a-5p联合CEA、NSE、CYFRA21-1诊断肺癌、肺鳞癌、肺腺癌、小细胞肺癌的AUC为0.849、0.647、0.748、0.773,除肺鳞癌外,联合检测的AUC均高于单项检测。结论肺癌患者血清miR-135a-5p水平高于肺部良性疾病患者和健康对照者,其与其他肿瘤标志物联合检测可提高肺癌及其不同病理类型的诊断效能。 Objective To measure the expression level of serum miR-135a-5p in lung cancer tissues and its diagnostic value.Methods Serum samples were collected from 115 patients with lung cancer(lung cancer group),100 patients with benign lung diseases(benign group)and 100 healthy persons(control group).Serum levels of miR-135a-5p,carcinoembryonic antigen(CEA),neuron-specific enolase(NSE),cytokeratin 19-fragments(CYFRA21-1)were detected.Receiver operating curve(ROC)was performed to evaluate the auxiliary diagnostic efficacy of miR135a-5p,CEA,NSE and CYFRA21-1 in lung cancer and its different pathological types.Results The median level of serum CEA(4.68ng/ml vs 3.89 ng/ml vs 2.72ng/ml),NSE(15.27ng/ml vs 14.50ng/ml vs 11.75ng/ml),CYFRA21-1(5.03ng/ml vs 4.27ng/ml vs 2.93ng/ml),and miR-135a-5p(2.83ng/ml vs 1.21ng/ml vs 1.06ng/ml)in lung cancer patients were significantly higher than those in benign lung disease patients and healthy controls(all P<0.05).The serum level of miR-135a-5p in lung cancer patients was significantly correlated with the pathological type,lymph node metastasis and clinical stage(all P<0.05),which were not correlated with gender and tumor diameter(both P>0.05).The AUC of combined detection of serum miR-135a-5p,CEA,NSE,and CYFRA21-1 in the diagnosis of lung cancer,lung squamous cell carcinoma,lung adenocarcinoma and small cell lung cancer was 0.849,0.647,0.748,and 0.773,respectively,which was significantly higher than that of a single detection in lung cancer,lung adenocarcinoma and small cell lung cancer.Conclusion The serum level of miR-135a-5p in lung cancer patients is higher than that in patients with benign lung diseases and healthy controls.Detection of it combined with other tumor makers can improve the diagnostic efficiency of lung cancer and its different pathological types.
