集束化干预对全胸腔镜肺叶切除术后合并气胸患者康复的影响研究

Effects of bundles of care on rehabilitation of patients complicated with pneumothorax after total thoracoscopic lobectomy

目的探讨采用集束化护理对全胸腔镜肺叶切除术后合并气胸患者进行干预,对其康复的影响。方法在2019年1月至2021年6月医院接受全胸腔镜肺叶切除术的肺癌患者中,选取术后合并气胸并满足研究纳入条件的76例患者为对象;按照随机数字表法分组原则,将患者分为对照组和研究组。其中,对照组患者行常规护理干预,研究组则采取集束化护理干预。观察2组康复情况,同时比较观察2组干预前后肺功能指标水平,统计患者并发症发生率,并对患者进行护理满意度调查。结果经护理后比较,研究组术后胸管留置、氧疗、进食、下床活动以及住院等时间均短于对照组,疼痛视觉模拟评分(VAS)低于对照组(P<0.05);用力肺活量(FVC)与1 s用力呼气容积(FEV1)/FVC水平高于对照组(P<0.05);研究组护理满意度(92.11%)高于对照组(73.68%)(P<0.05),2组并发症发生率比较,差异无统计学意义(P>0.05)。结论集束化干预在全胸腔镜肺叶切除术后合并气胸患者护理中,对促进患者康复进程、改善肺功能有积极作用,患者认可度较高,有一定的临床应用价值。

Objective To explore the effects of bundles of care on the rehabilitation of patients complicated with pneumothorax after total thoracoscopic lobectomy(TTL).Methods From January 2019 to June 2021,76 patients with lung cancer who underwent TTL and complicated with pneumothorax after surgery were randomly assigned into the control group and the study group,in which they received conventional nursing intervention and bundles of care,respectively.The key observation was rehabilitation,pulmonary function index,incidence of complications,nursing satisfactory degree.Results After nursing,the postoperative duration of chest tube indwelling,oxygen therapy,feeding,ambulation and hospitalization,and the visual analog scale(VAS)for pain in the study group were decreased compared with the control group(P<0.05).Forced vital capacity(FVC)and forced expiratory volume in one second(FEV1)/FVC level in the study group were higher than those in the control group(P<0.05).The nursing satisfactory degree in the study group was higher than that in the control group(92.11%vs 73.68%,P<0.05).The difference was not statistically significant in the incidence of complications between groups(P>0.05).Conclusion For patients complicated with pneumothorax after TTL,bundles of care can actively promote the recovery of patients,improve lung function,and with a high degree of recognition from patients.It has certain clinical application value.
