基于经筋理论探讨针刀治疗早期膝骨性关节炎

Acupotomy in the Treatment of Early Knee Osteoarthritis Based on the Theory of Meridian Tendon

目的 研究针刀疗法对早期膝骨性关节炎患者临床效果、疼痛和本体感觉功能的影响。方法 选取2021年3月—2022年2月于福建中医药大学附属康复医院门诊就诊的早期膝骨性关节炎患者68例,按信封法随机分为2组,2组均以常规手法治疗为基础,治疗组34例配合针刀治疗,对照组34例配合普通针刺治疗。分别于治疗前后对2组进行效果评价、疼痛视觉模拟量表(VAS)评分、本体感觉功能评估。结果 治疗后,2组VAS评分、本体感觉功能比治疗前明显改善,差异有统计学意义(P <0.05)。而且,在VAS评分、本体感觉功能评估中,治疗组的效果优于对照组,差异有统计学意义(P <0.05)。治疗组的总有效率为85.29%(29/34),高于对照组的58.82%(20/34),差异有统计学意义(P <0.05)。结论 针刀疗法可以减轻早期膝骨性关节炎患者的疼痛,提高下肢的神经肌肉控制能力,在临床上的进一步推广应用和研究将有利于膝骨性关节炎患者的功能康复。

Objective To study the therapeutic effect of acupotomy on clinical effect,pain and proprioceptive function in patients with early knee osteoarthritis.Methods A total of 68 patients with early knee osteoarthritis who were treated in Rehabilitation Hospital Affiliated to Fujian University of Traditional Chinese Medicine from March 2021 to February 2022 were randomly divided into 2 groups according to the envelope method.The two groups were treated on the basis of conventional manipulation,34 cases in the treatment group were treated with acupotomy,and 34 cases in the control group were treated with ordinary acupuncture.Efficacy evaluation,visual analogue scale(VAS)score and proprioceptive function evaluation were performed before and after treatment.Results After treatment,VAS score and proprioceptive function of 2 groups were significantly improved compared with that before treatment,and the difference was statistically significant(P<0.05).Moreover,in VAS score and proprioceptive function evaluation,the effect of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the treatment group was 85.29%(29/34),which was higher than that of the control group(58.82%,20/34),and the difference was statistically significant(P<0.05).Conclusion Acupotomy therapy can relieve the pain of patients with early knee osteoarthritis and improve the neuromuscular control ability of lower limbs.Further application and research in clinical practice will be beneficial to the functional rehabilitation of patients with knee osteoarthritis.
