摘要
目的:探讨外耳、中耳疾病导致儿童传导性听力损失的CT特征。方法:回顾性分析2016年07月至2020年12月行CT检查的126例患儿的临床及CT资料。结果:126例患儿中,炎症74例最多见,其中中耳乳突炎66例(急性45例、慢性21例),另有7例外耳道胆脂瘤,1例恶性外耳道炎;先天畸形35例,其中外耳道畸形24例,伴随听骨链畸形21例,单纯中耳畸形5例,卵圆窗及圆窗闭塞共6例,合并面神经走行异常6例,伴随综合征表现8例;听小骨创伤共12例,其中8例合并颞骨骨折;肿瘤及肿瘤性疾病共5例,其中骨纤维结构不良和朗格汉斯细胞组织细胞增生症各2例、横纹肌肉瘤1例。结论:CT对儿童传导性听力损失患儿,能提供准确的诊断,对临床制订正确的治疗方案提供重要的参考依据。
Objective:To investigate CT features of conductive hearing loss in children caused by external and middle ear diseases.Methods:The clinical and CT data of 126 children who underwent CT from July 2016 to December 2020 were analyzed retrospectively.Results:Among the 126 children,74 cases were inflammatory,which was the most common,including 66 cases of middle ear mastoiditis(acute 45 cases,chronic 21 cases),7 cases of auditory canal cholesteatoma and1 case of malignant external auditory canal inflammation;There were 35 cases of congenital malformations,including 24 cases of extermal auditory canal malformation,21 cases of ossicular chain malformation,5 cases of simple middle ear malformation,6 cases of oval window and round window occlusion,6 cases of abnormal facial nerve routing and 8 cases of concomitant syndrome;There were 12 cases of auditory bone trauma,of which 8 cases were complicated with temporal bone fracture;There were 5 cases of tumor and neoplastic diseases,including 2 cases of bone fibrous dysplasia,2 cases of Langerhans cell histiocytosis and 1 case of rhabdomyosarcoma.Conclusion:CT can provide accurate diagnosis for children with conductive hearing loss,and provide important reference basis for elinical treatment.
