血管内皮生长因子A基因多态性与老年2型糖尿病患者院内肺部感染的相关性

Effect of vascular endothelial growth factor A gene polymorphism on hospitalized pulmonary infection in elderly patients with type 2 diabetes mellitus

摘要 目的探讨血管内皮生长因子A(VEGF-A)基因多态性对老年2型糖尿病(T2DM)患者院内肺部感染的影响。方法选择2021年3月-2022年3月于淮安市第四人民医院内科因T2DM住院的老年患者为研究对象,选择住院期间发生肺部感染和未发生肺部感染者各45例,分别纳入感染组和非感染组。所有患者均在入院当天抽取外周静脉血并提取全血DNA,以聚合酶链式扩增反应(PCR)扩增目的片段序列,并以2%琼脂糖凝胶电泳分析基因分型结果。通过电子病历系统获取所有T2DM患者性别、年龄、糖尿病病程、合并高血压和合并糖尿病情况;统计感染组患者感染类型、感染严重程度和抗感染治疗时间。结果感染组、非感染组VEGF-A基因rs1547651位点各基因型实际频数和理论频数分布比较差异均无统计学意义(χ^(2)=0.852、1.874,P=0.653、0.392)。感染组VEGF-A基因rs1547651位点携带AA基因型(17.78%vs.48.89%)和A等位基因频率(34.44%vs.63.33%)低于非感染组,携带TT基因型(48.89%vs.22.22%)和T等位基因(65.56%vs.36.67%)频率高于非感染组,差异均有统计学意义(P均<0.05)。感染组VEGF-A基因rs1547651位点携带TT基因型患者和携带AA/AT型患者感染类型和抗感染治疗时间比较,差异均无统计学意义(P均>0.05),而两组感染严重程度比较差异有统计学意义(χ^(2)=0.876,P=0.026)。结论VEGF-A基因rs1547651位点携带TT基因型增加T2DM老年患者住院期间肺部感染风险,且与感染严重程度有关。 Objective To investigate the effect of vascular endothelial growth factor A(VEGF-A)gene polymorphism on hospitalized pulmonary infection in elderly patients with type 2 diabetes(T2DM). Methods Elderly patients with T2DM in the internal medicine department of our hospital were selected from March 2021 to March 2022 as the research subjects,45patients with pulmonary infection and 45 patients without pulmonary infection,and they were included in the infection group and the non-infection group,respectively. Peripheral venous blood was drawn from all patients on the day of admission and whole blood DNA was extracted;the target fragment sequence was amplified by polymerase chain amplification reaction(PCR),and the genotyping results were analyzed by 2% agarose gel electrophoresis. The gender,age,duration of diabetes,hypertension and diabetes of all T2DM patients were obtained through the electronic medical record system;of the infection type,infection severity and anti-infection treatment time of the patients in the infection group were counted.Results There was no significant difference between the actual frequency distribution and the theoretical frequency distribution of each genotype at the rs1547651 locus of VEGF-A gene between the infection group and the non-infection group(χ^(2)=0.852,1.874,P=0.653,0.392). The frequencies of AA genotype(17.78% vs. 48.89%)and A allele(34.44% vs.63.33%)at rs1547651 site of VEGF-A gene in the infected group were lower than those in the non-infected group;the frequencies of TT genotype(48.89% vs. 22.22%)and T allele(65.56% vs. 36.67%)were higher than those in the noninfected group,and the differences were statistically significant(all P<0.05). There were no significant differences in infection type and anti-infection treatment time between patients with TT genotype at the rs1547651 locus of VEGF-A gene and patients with AA/AT type in the infection group(P<0.05),but there was a significant difference in the severity of infection between the two groups(χ^(2)=0.876,P=0.026). Conclusion Carrying the TT genotype at rs1547651 of the VEGF-A gene could increases the risk of pulmonary infection during hospitalization in elderly patients with T2DM and was associated with infection severity.
