早期开髓减压引流术对急性牙髓炎患者的影响

Effect of Early Open Pulp Decompression and Drainage Operation on Patients with Acute Pulpitis

目的:探究早期开髓减压引流术对急性牙髓炎患者的影响。方法:选取2021年7月—2022年7月邵武市立医院收治的102例急性牙髓炎患者为研究对象。根据治疗方法差异将其分为保髓组(n=51)和减压组(n=51)。保髓组给予保髓疗法,减压组给予早期开髓减压引流术。比较两组临床疗效,治疗前后炎症指标、细菌菌落、疼痛程度及并发症。结果:减压组总有效率为94.12%,明显高于保髓组的78.43%,差异有统计学意义(P<0.05)。治疗后,减压组白细胞介素-8(interleukin-8,IL-8)及白三烯B4(leukotriene B_(4),LTB_(4))及P物质(substance P,SP)水平均明显低于保髓组,差异有统计学意义(P<0.05)。治疗后,减压组粪肠球菌、具核梭杆菌及牙髓卟啉单胞菌菌落数量均明显低于保髓组,差异有统计学意义(P<0.05)。治疗后,减压组视觉模拟评分法(VAS)评分明显低于保髓组,差异有统计学意义(P<0.05)。两组并发症发生率比较差异无统计学意义(P>0.05)。结论:急性牙髓炎患者进行早期开髓减压引流术治疗可有效降低炎症反应,同时减少患者口腔内细菌含量,有效改善疼痛程度。

Objective:To explore the effect of early open pulp decompression and drainage operation on patients with acute pulpitis.Method:A total of 102 patients with acute pulpitis admitted to Shaowu Municipal Hospital from July 2021 to July 2022 were selected as the research objects.According to the differences in treatment methods,they were divided into pulp preservation group(n=51)and decompression group(n=51).The pulp preservation group was treated with pulp preservation therapy,and the decompression group was treated with early open pulp decompression and drainage operation.The clinical efficacy,inflammatory index,bacterial colony,pain degree before and after treatment and complications were compared between the two groups.Result:The total effective rate of decompression group was 94.12%,which was significantly higher than that of pulp preservation group(78.43%),and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-8(IL-8),leukotriene B4(LTB4)and substance P(SP)in the decompression group were significantly lower than those in the pulp preservation group(P<0.05).After treatment,the number of bacterial colonies of Enterococcus faecalis,Fusobacterium nucleatus and Porphyromonas medullae in the decompression group were significantly lower than those in the pulp preservation group,and the differences were statistically significant(P<0.05).After treatment,the visual analogue scale(VAS)score of decompression group was significantly lower than that of pulp preservation group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Early open pulp decompression and drainage operation treatment for patients with acute pulpitis can effectively reduce the inflammatory response,reduce the content of bacteria in the oral cavity,and effectively improve the degree of pain.
