米非司酮联合八草止血汤对异常子宫出血患者的影响

Effect of Mifepristone Combined with Bacao Zhixue Decoction on Patients with Abnormal Uterine Bleeding

目的:分析米非司酮联合八草止血汤对异常子宫出血患者的影响。方法:回顾性选取2020年6月—2022年6月南靖县妇幼保健院收治的208例异常子宫出血患者的资料。根据不同用药将其分为对照组及观察组,各104例。两组入院后均行常规治疗,对照组给予米非司酮片,观察组在对照组基础上联合八草止血汤。比较两组临床疗效,用药前后中医症候积分、性激素及子宫内膜厚度、子宫动脉血流动力学,不良反应和复发率。结果:观察组总有效率高于对照组,不良反应发生率与复发率均低于对照组,差异有统计学意义(P<0.05)。用药后,观察组阴道出血天数评分、经色与经质评分、烦热面赤评分、排便不爽评分、小腹疼痛评分及总分均低于对照组,差异有统计学意义(P<0.05)。用药后,观察组促黄体生成素、卵泡刺激素、孕酮、雌二醇水平、子宫内膜厚度均低于对照组,差异有统计学意义(P<0.05)。用药后,观察组子宫动脉收缩期峰值血流速度、舒张末期血流速度、搏动指数均低于对照组,阻力指数高于对照组,差异有统计学意义(P<0.05)。结论:予以异常子宫出血患者米非司酮联合八草止血汤治疗,可提高止血效果,缓解病情,对性激素和血流动力学均有改善作用,也可减少不良反应,降低复发率。

Objective:To analyze the effect of Mifepristone combined with Bacao Zhixue Decoction on patients with abnormal uterine bleeding.Method:The data of 208 patients with abnormal uterine bleeding in Nanjing County Maternal and Child Health Care Hospital from June 2020 to June 2022 were retrospectively selected.According to different drugs,they were divided into control group and observation group,104 cases in each group.Both groups were given routine treatment after admission.The control group was given Mifepristone Tablets,and the observation group combined with Bacao Zhixue Decoction on the basis of the control group.The clinical efficacy,traditional Chinese medical science symptom score,sex hormone and endometrial thickness,uterine artery hemodynamics before and after medication,adverse reactions and recurrence rate were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group,and the incidence of adverse reactions and recurrence rate were lower than those of the control group,and the differences were statistically significant(P<0.05).After medication,the score of vaginal bleeding days,menstrual color and menstrual quality,irritated heat and face redness,difficult defecation,lower abdominal pain and total score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After medication,the levels of luteinizing hormone,follicle stimulating hormone,progesterone and estradiol and endometrial thickness in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After medication,the peak systolic stage blood flow velocity,end diastolic stage blood flow velocity,and pulsatile index of the uterine artery in the observation group were lower than those in the control group,while the resistance index was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:The treatment of Mifepristone combined with Bacao Zhixue Decoction in patients with abnormal uterine bleeding can improve the hemostatic effect,relieve the condition,it has the improvement effect on the sex hormones and hemodynamics,can reduce adverse reactions,reduce the recurrence rate.
