摘要 目的:探究血清肌酐(Scr)及纤维蛋白原(FIB)在2型糖尿病视网膜病变增殖期中的应用价值。方法:选取2021年1月—2022年1月福建省立医院南院收治的122例2型糖尿病视网膜病变患者为研究对象。根据是否为2型糖尿病视网膜病变增殖期将其分为对照组(61例,非增殖期)及研究组(61例,增殖期)。所有患者检测Scr、FIB水平。收集两组一般资料。比较两组Scr、FIB水平及一般资料。并对2型糖尿病视网膜病变增殖期的影响因素进行logistic回归分析。分析Scr、FIB对2型糖尿病视网膜病变增殖期的诊断价值。结果:研究组Scr、FIB水平均高于对照组,差异有统计学意义(P<0.05)。两组性别、年龄、饮酒史、高血压史、吸烟史、冠心病史、空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)水平比较差异无统计学意义(P>0.05)。多因素分析结果显示,Scr及FIB水平均为2型糖尿病视网膜病变增殖期的危险因素(P<0.05)。ROC分析显示,Scr、FIB诊断2型糖尿病视网膜病变增殖期的最佳截断点分别为132.52μmol/L、3.21 g/L,Scr、FIB两者联合的特异度、AUC均高于Scr、FIB单独检测。结论:Scr、FIB水平与2型糖尿病视网膜病变增殖期存在一定的关系,且Scr、FIB联合检测对2型糖尿病视网膜病变增殖期的诊断效能较高,可作为临床评估2型糖尿病视网膜病变增殖期的重要参考指标。 Objective:To explore the application value of serum creatinine(Scr)and fibrinogen(FIB)in proliferative phase of type 2 diabetic retinopathy.Method:A total of 122 patients with type 2 diabetic retinopathy admitted to the Fujian Provincial Hospital South Hospital from January 2021 to January 2022 were selected as the research objects.According to whether it is the proliferative phase of type 2 diabetic retinopathy,the patients were divided into control group(61 cases,non-proliferative stage)and study group(61 cases,proliferative stage).Scr and FIB levels were detected in all patients.The general information of the two groups were collected.Scr,FIB levels and general data were compared between the two groups.logistic regression analysis was performed to analyze the influencing factors of proliferative phase of type 2 diabetic retinopathy.The diagnostic value of Scr and FIB on proliferative phase of type 2 diabetic retinopathy was analyzed.Result:The levels of Scr and FIB in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in gender,age,drinking history,hypertension history,smoking history,coronary heart disease history,fasting blood glucose(FPG),total cholesterol(TC),triglycerides(TG)levels between the two groups(P>0.05).The results of multivariate analysis showed that both Scr and FIB levels were risk factors for proliferative phase of type 2 diabetic retinopathy(P<0.05).ROC analysis showed that the best cut-off points of Scr and FIB for the diagnosis of proliferative phase of type 2 diabetic retinopathy were 132.52μmol/L and 3.21 g/L respectively.The specificity and AUC of the combination of Scr and FIB were higher than those of Scr and FIB alone detection.Conclusion:There is a certain relationship between the levels of Scr and FIB and the proliferative phase of type 2 diabetic retinopathy,and the combined detection of Scr and FIB has a high diagnostic efficiency for the proliferative phase of type 2 diabetic retinopathy,which can be used as an important reference index for clinical evaluation of the proliferative phase of type 2 diabetic retinopathy.
