Huo Qin′s Differentiation and Treatment of Dry Eye Disease Based "One-Qi Circumfluence" Theory

干眼症发病的核心病机为清阳下陷,浊阴逆上。针对木郁土壅、金水不生、水寒土湿引起的气机升降失常,清浊反作导致的干眼症,临症运用“一气周流,土枢四象”理论,以健运脾胃、斡旋中气为核心,审视四象之气的动态变化,恢复一体化元气周流,采用培己土达乙木而养肝血以润达于目、行戊土生金水而敛肺阴以泽溉于目、运中气温癸水而理清浊以复升降之枢等治则,在黄元御治方的基础上辨证用药,为治疗干眼症提供临床诊疗思路。

The core pathogenesis of dry eye disease(DED)is the Yang sinking and the turbid Yin reversal.In the clinical treatment of DED,which is induced by abnormal Qi movement caused by Wood and Earth stagnation,abnormal production of Gold and Water,Water coldness and Earth dampness as well as the reversed clear Qi and turbidity Qi,"One-Qi circumfluence and Earth being the pivots other four elements""theory should be based on.Focusing on invigorating the Spleen and Stomach and mediating the middle Qi,the Qi changes of the other four elements should be examined and the integrated Yuan Qi circumfluence should be restored.The therapeutic principles such as cultivating the Earth and regulating the Liver Qi to tonify blood and moisten the eyes,helping with the Earth and generating Water to astringe the Lung Yin so as to irrigate the eyes,helping with Zhong Qi and warming Water to clear the turbidity and recover the ascending and descending pivots should be followed.In syndrome differentiation and medication Huang Yuanyu′s prescription,which provides clinical diagnosis and treatment ideas for the DED treatment,can be good examples.
