温阳活血法对浆细胞性乳腺炎疼痛介质、泌乳素及血清炎性因子的影响

Effects of Warming Yang and Activating Blood Circulation Method on Pain Mediators,Prolactin and Serum Inflammatory Factors in Plasma Cell Mastitis

目的:探讨温阳活血法对浆细胞性乳腺炎疼痛介质、泌乳素及血清炎性因子的影响。方法:将120例浆细胞性乳腺炎患者作为本次前瞻性研究的研究对象,分为对照组和试验组,均为60例。两组患者入院均行糖皮质激素联合手术区段切除治疗,试验组在此基础上口服温阳活血汤进行治疗。两组患者疗程均为20 d。治疗结束后统计疗效。对比对照组和试验组治疗前后各项临床症状积分、血清P物质(substance P,SP)、多巴胺(dopamine,DA)、泌乳素(prolactin,PRL)、白细胞介素-1β(interleukin-1β,IL-1β)、IL-6和肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)含量。结果:治疗后,试验组肿块直径、患处肤色、疼痛程度、乳头溢液、同侧腋窝淋巴结肿大、体温升高等临床症状积分相较于对照组显著降低(P<0.05);试验组有效率95.00%优于对照组81.67%(P<0.05);治疗后,试验组血清SP、PRL、IL-1β、IL-6、TNF-α含量相较于对照组显著降低(P<0.05),试验组血清DA含量相较于对照组显著升高(P<0.05);对照组和试验组3个月、6个月、12个月内复发情况比较无明显差异(P>0.05)。结论:温阳活血法对浆细胞性乳腺炎患者疗效较好,有助于缓解患者疼痛症状,通过增加多巴胺的分泌降低泌乳素含量,并减少血清炎性因子的分泌,缓解机体炎症。

Objective:To investigate the effect of warming Yang and activating Blood circulation on pain mediators,prolactin,and serum inflammatory factors in plasma cell mastitis.Methods:120 patients with plasma cell mastitis were selected as the research objects of this prospective study.The case collection time was from January 2020 to January 2022.All included cases were screened according to the inclusion and exclusion criteria.120 subjects were divided into control group and experimental group,each with 60 cases.Both groups were treated with glucocorticoid combined with surgical segmental resection,and the experimental group was treated with Wenyang Huoxue Decoction on this basis.The treatment course of the two groups was 20 days.The curative effect was counted after the treatment.The scores of clinical symptoms,serum substance P(SP),dopamine(DA),prolactin(PRL),interleukin-1β(IL-1β),IL-6,and tumor necrosis factor-α(TNF-α)content were compared between the control group and the experimental group before and after treatment.Results:After treatment,clinical symptom scores such as tumor diameter,the skin color of affected area,pain degree,nipple discharge,ipsilateral axillary lymph node enlargement,and elevated body temperature in the experimental group were significantly lower than those in the control group(P<0.05).The effective rate of the experimental group was 95.00%,which is better than that of the control group at 81.67%(P<0.05).After treatment,the levels of serum SP,PRL,IL-1β,IL-6,and TNF-αin the experimental group were significantly lower than those in the control group(P<0.05),and the serum DA levels in the test group were significantly higher than those in the control group(P<0.05).There was no significant difference between the control group and the experimental group in terms of recurrence within 3 months,6 months,and 12 months(P>0.05).Conclusion:The method of warming Yang and activating Blood circulation has a better effect on patients with plasma cell mastitis,which can help relieve the pain level of patients,reduces the content of prolactin by increasing the secretion of dopamine,and reduces the secretion of serum inflammatory factors,so as to relieve the inflammation of the body.
